Objective: To investigete the relationship between pre-pregnancy body mass index(BMI)and gestation weight gain(GWG)to preganancy complication and adverse outcomes. Methods: A retrospective analysis of 2720 cases of regular birth examination and delivery in the First Affiliated Hospital of Xinjiang Medical University included 890 cases of primiparas of the right age.The case data were grouped according to maternal pre-pregnancy body mass index and gestation weight gain.To analyze the influence of pre-pregnancy body mass index, gestation weight gain, age and cesarean section rate, pregnancy complications and adverse outcomes of newborns. Results: Pre-pregnancy BMI was overweight and obesity increased the cesarean section rate(OR = 2.46,5.20),increased the risk of gestational diabetes mellitus(OR = 1.86,1.33),hypertensive disorder complicating pregnancy(OR = 4.07,4.53),fetalmacrosomia(OR = 5.72,2.87),premature delivery(OR = 2.15,9.19)and postpartum hemorrhage(OR = 1.44,1.85).Gestation weight gain exceeds IOM’s recommended weight gain range, increasing the risk of cesarean section(OR = 2.93),gestational diabetes mellitus(OR = 1.10),hypertensive disorder complicating pregnancy(OR = 1.50),fetalmacrosomia(OR =1.15),amniotic fluid pollution(OR = 4.20),and the age of 36-40 gravidas increasing the risk of cesarean section(OR = 6.42),gestational diabetes mellitus(OR = 4.89),hypertensive disorder complicating pregnancy(OR = 3.98),premature rupture of membranes(OR = 2.48),and postpartum hemorrhage(OR = 1.22).Conclusion:Overweight or obesity of body mass index during pregnancy, excessive weight gain during pregnancy and advanced age can increase cesarean section rate, leading to adverse outcomes for mothers and infants. Therefore,it is particularly important to control weight and gain reasonable weight during pregnancy.Clinically, the weight of pregnant womencan be controlled according to the weight range recommended by IOM,and health education and weight follow-up during pregnancy can be strengthened. It is expected that through multi-party investigation, the standard of weight gain range suitable for each stage of pregnancy in all regions of China can be obtained as soon as possible.
