Objective:To explore the parameter range and diagnostic value of perfusion fraction,pure diffusion coefficient and pseudodiffusion conefficient in patients with spinal tuberculosis patients by diffusion-weighted imaging with incoherent motion in magnetic resonance voxel.Methods:Eighteen patients in the spinal tuberculosis group and the normal control group were collected,and 36 subjects underwent 3.0T MRI routine and IVIM-DWI sequence scanning.Using the double-index model post-processing software,the IVIM-DWI quantitative f,D,D of the spinal tuberculosis group(the diseased vertebral body,the jumping 2 vertebral body,the intervertebral disc)and the corresponding normal control group(normal vertebral body,intervertebral disc)were obtained.Parametric indicators,the value of the IVIM-DWI parameters(f,D,D*)in the spinal tuberculosis group and the normal control group,and the value of the diagnosis of spinal tuberculosis..Results:(1)The f value of the vertebral body in the spinal tuberculosis group was[12.91(8.15,22.73)%],which was lower than that in the control group[37.16(30.45,47.07)%];(Z=6.841,P<0.001);D value[0.88(0.73,1.40)x10-3mm2/s],D*value[39.99(20.15,66.35)x10-3mm2/s],significantly higher than normal vertebral body D value[0.07(-0.12,0.28)x10-3mm2/s],D*value[20.37(12.26,29.97)x10-3mm2/s];(Z=7.598,P<0.001;t=3.842,P<0.001).(2)Compared with the normal vertebral body f value of the vertebral body of the spinal tuberculosis group,the former was lower than the latter[40.51(33.75,46.28)%],(Z=3.421,P=0.001);the D values were significantly higher than the former[0.05(-0.20,0.15)x10-3mm2/s],(Z=3.743,P<0.001).(3)In the spinal tuberculosis group,the intervertebral disc f value[6.72(4.36,11.53)%],D value[2.06(1.92,2.26)x10-3mm2/s],were higher than the normal intervertebral disc of the control group[5.72(3.00,7.85)%],[1.88(1.79,1.85)x10-3mm2/s];(Z=2.276,P=0.023)、(Z=3.919,P<0.001).The maximum area under the curve for obtaining the D value of the ROC curve is 0.960,the sensitivity is 95.74%,the specificity is 87.56%,and the optimal diagnostic threshold is 0.63x10-3 mm2/s.Conclusion:IVIM-DWI can quantitatively evaluate the diffusion and microvascular perfusion characteristics of diseased water molecules.Through the range of quantitative parameters of f,D and D*in IVIM,D value has the highest sensitivity and specificity for diagnosis of spinal tuberculosis,and provides clinical diagnosis of spinal tuberculosis.A valid range of referencevalues. |
