Objective:To analyze the relationship between serum bilirubin and the severity of coronary atherosclerotic heart disease,and to explore the mechanism of serum bilirubin in coronary atherosclerotic heart disease.Methods: A total of 236 patients who underwent coronary angiography in the Department of Cardiology in People's Hospital of Liaoning Province from January 2017 to July 2018 were retrospectively selected according to the inclusion and exclusion criteria.Two experienced cardiologists used the same computer imaging system to determine whether the coronary artery and its large branch vessel diameter were ?50%,and were divided into coronary heart disease group and non-coronary heart disease group,including 183 cases of coronary heart disease group and 53 cases of non-coronary heart disease group.There were 110 male and 73 female in coronary heart disease group,29 males and 23 females in non-coronary heart disease group.Then,according to the results of coronary angiography,the SYNTAX official website calculator was used to score each patient with confirmed coronary heart disease.First,according to the origin of the posterior descending bronchus,the left and right dominant types are distinguished,and then the lesions of the main branches of the LM,LAD,LCX and the RCA are weighted,and special lesions such as calcification,thrombosis,bifurcation lesions,open lesions and diffuse lesions were added.The components of coronary heart disease were low group(0-22),middle group(23-32),and high group(>33),Among them,there were 116 cases of low group including 62 cases of male and 54 cases of female;48 cases of middle group including 33 cases of male and 15 cases of female;19 cases of high group including 15 cases of male and 4 cases of female.The expressions of serum bilirubin,C-reactive protein and other clinical biochemical indicators in the non-coronary heart disease group and the low,middle and high group of coronary heart disease were analyzed and compared.The data were analyzed by SPSS 23.0 statistical software.Results: 1.Serum total bilirubin was 13.42±2.56umol/L in the non-coronary heart disease group,12.97±2.07 umol/L in the lowscore group,10.74±2.65 umol/L in the middlescore group,and 9.79±2.24 umol/L in the high score group.The difference between the four groups was statistically significant(p<0.0001).2.Serum indirect bilirubin was 10.04±2.72umol/L in the non-coronary heart disease group,9.53±2.55 umol/L in the low score group,7.77±2.15umol/L in the middlescore group,and 7.53±2.40 umol/L in the high score group.The difference between the four groups was statistically significant(p<0.0001).3.Serum direct bilirubin was 2.66±1.31 umol/L in the non-coronary heart disease group,2.45±1.34 umol/L in the low score group,2.50±1.53 umol/L in the middlescore group,and 2.27±0.99 umol/L in the high score group.The difference between the four groups was not statistically significant(p>0.05).4.C-reactive protein was 1.53±1.85mg/L in the non-coronary heart disease group,2.11±2.93 mg/L in the lowscore group,3.93±5.86 mg/L in the middlescore group,and 3.99±5.76 mg/L in the high score group.The difference between the four groups was statistically significant(p<0.05).5.The troponin T,serum total protein,serum albumin,serum creatinine,serum triglyceride,serum low density lipoprotein,left ventricular ejection fraction,left ventricular diameter were statistically different among the four groups(p<0.05).6.Spearman correlation analysis showed that there was a negative correlation between SYNTAX score and TBIL,IBIL,TP,ALB and EF(p<0.05);there was a positive correlation with CRP and TNT-HS(p<0.05).Correlation analysis also found that there was no correlation between CRP and TBIL,DBIL(p>0.05),and a negative correlation between CRP and IBIL(p<0.05).Conclusion: 1.Serum total bilirubin and indirect bilirubin levels are related to the severity of coronary atherosclerotic heart disease.The more severe the lesion,the lower the serum bilirubin level.2.C-reactive protein is related to the severity of coronary atherosclerotic heart disease.The more severe the lesion,the higher the C-reactive protein.
