Purpose:Some patients with arrhythmia without the history of headache have undergone radiofrequency ablation of transseptal puncture(TSP).and new-onset migraine-like headaches after surgery.to explore the factors that cause this headache.Methods:42 patients with new-onset migraine-like headache after TSP were selected as the headache group.42 patients with no headache after TSP were selected as non-headache group according to 1:1 case-controlled.The two groups of case information were collected,and their RLSs tested by contrast-enhanced transcranial Doppler ultrasound(cTCD)after TSP were compared.The patients in the headache group were followed up for 3 months,and the headache information was collected,and their situations of RLS were reviewed in the third month by cTCDResults:Of the 1327 patients,42(42/1327.3.17%)had a new-onset migraine-like headache with an age of 48.79 ± 10.18 years.Of the headache group,28 people with smoking and 22 people with a family history of headache were significantly higher than the non-headache group respectively(P<0.05).and the anxiety self-rating scale(SAS)score of 59.43±10.92 and the Athens Insomnia Scale(AIS)of 5.34±3.99 were significantly worse than the non-headache group(P<0.05).In the results of cTCD examination of headache group.27 persistent types were significantly high(P<0.05).and large RLS patients were significantly more than non-headache group(P<0.05).24 patients with persistent type in the non-headache group were significantly more(P<0.05).In review of cTCD test at 3rd month after TSP,the headache group showed a significant decrease in 19 patients with cTCD positive than before(P<0.05).and the patients with large RLS were also significantly reduced(P<0.05).Higher SAS scores(P<0.05).smoking history(P<0.05),family history of headache(P<0.05).and large RLS(P<0.05)are independent risk factors in postoperative patients occurred migraine-like headaches following TSP.During the follow-up of headache group,the number of monthly headaches gradually decreased,and the number of headaches was in persistent types higher than potential type(P<0.05).The frequency and duration of headache with large RLS were more serious than those with small RLS(P<0.05),and visual aura was more likely to occur in large RLS after TSP(P<0.05).Conclusion:The new-onset migraine-like headache after TSP is related to postoperative RLS,especially in patients with large RLS.The patient is female,smoking,family history of headache,multiple puncture,anxiety state and sleep disorder may occur new-onset migraine-like headache more easily after TSP.New-onset migraine-like headache patients with large RLS after surgery have longer headache duration and more frequency,and may be more prone to visual aura.
