Clinical Observation Of Autologous Tooth Bone Graft Material Used At Different Sets In The Maxilla

Objective: to observe the short-term clinical effect of autologous dental bone powder used as bone grafting material in maxillary bone deficiency patients,and to observe the stability of its osteogenic effect in different anatomical sites of maxilla.Methods: from January 2017 to October 2018,9 patients aged 27-55 years who received implant surgery in the stomatological hospital affiliated to Dalian Medical University and used autologous dental bone powder as the bone graft material to cover the PRF membrane were selected in this study.A total of 14 implants(all Thommen implants)were implanted in the same period or later.Among them,there were 5 patients with sufficient height and insufficient width of bone in the anterior teeth area for bone grafting(including 2 patients who underwent osteotomy),8 implants were implanted,4 patients with sufficient width and insufficient height of bone in the upper posterior teeth area for maxillary sinus elevation and bone grafting,and 6 implants were implanted.Two cases were implanted immediately and one case was delayed after bone grafting.CBCT imaging examinations were performed preoperatively,3 months after surgery and 12 months after surgery,and the imaging data were measured and analyzed.Results :(1)no infection,inflammatory exudation or perforation of maxillary sinus occurred in all patients.(2)there was no loosening or shedding of the 14 implants,and the retention rate was 100%.(3)the average preoperative bone width of the maxillary anterior teeth was 3.75 mm,the average postoperative width was 7.38 mm,and the average postoperative height and width was 7.02 mm,all of which were significantly improved compared with that before surgery(P<0.05).The mean preoperative bone height of the maxillary posterior teeth was 5.09 mm,the mean postoperative height was 11.80 mm 3 months after surgery,and the mean postoperative height was 11.45 mm 12 months after surgery,which was significantly improved compared with that before surgery(P<0.05).(4)gingival papillary index was measured postoperatively,which was better than intraoperative filling.(5)nine patients showed tolerable reaction to the surgical process and postoperative response,and were basically satisfied with the postoperative repair effect.Conclusion: in this case study,it was found that autologous dental bone powder had good bone augmentation effect in patients with insufficient width of maxillary anterior teeth and insufficient height of maxillary posterior teeth,and the bone width or height in the operative area was significantly improved compared with that before surgery.The osteogenic stability of the maxillary anterior and posterior teeth was good.However,the long-term effects still need further follow-up observation of patients.
