Objective:To explore the clinical significance of platelet to lymphocyte ratio(PLR)and mean platelet volume(MPV)in the prediction of the gestational diabetesmellitus(GDM)。Methods:Selecting 214 cases of pregnant women who were the delivery in the Hospital of Yanbian University from September 30,2017 to September 30,2018,according to the ninth edition of Obstetrics and Gynecology advice,75g glucose tolerance assay(75g OGTT)is performed at 24-28 weeks of gestation and after 28 weeks of gestation.Fast blood-glucose(FBG)and blood-glucose after taking sugar for 1、2 hours are respectively under 5.1 mmol/L、10.0 mmol/L、8.5 mmol/L.According to this criterion that any blood-glucose reaches or exceeds the data above will be diagnosed as gestational diabetes mellitus(GDM),these pregnant women were divided into A、B and C three groups.A group is the normal pregnant women and works as the control group(n=82),B group is the diet control group which consists of those who were diagnosed as GDM but only needs to control diet,C group is the insulin group which consists of those who were diagnosed as GDM,but the diet control had no effect on them and needs to use insulin.Meantime we recorded the correlative factors(age、gestational age and weight of pregnant women)and blood cells(NEU、LYM、PLT and MPV)before the delivery and calculated PLR and recorded the neonatal weight.Finally to comparatively analyze the clinical significance of PLR、MPV and above correlative factors in the prediction of the gestational diabetes mellitus(GDM).Results:1.The age and weight of pregnant women in the group B and group C were both higher than those in the group A;the gestational age of pregnant women in the group B and group C were lower than that in the group A,which was all statistically significant(P<0.05);the infant weight in the group C was higher than it in the group B,with statistically significant difference(P<0.05);the gestational age and weight of pregnant women were positively correlated with neonatal weight.2.The NEU in the group B and group C were both higher than that in the group A,and group B was the highest,with statistical significance(P<0.05).The NEU was positively correlated with LYM and PLT;the area under the ROC curve for NEU was 0.756,which was statistically significant(P<0.05),the sensitivity、specificity and cut off value of NEU were 85.1%、55.0%、7.80.3.The LYM in the group B and group C were both lower than that in the group A,which had statistical significance(P<0.05).The area under the ROC curve for LYM was 0.298,which had no statistical significance(P>0.05).4.The PLT and MPV in the group B and group C had no statistical significance between those in the group A,the PLT was negatively correlated with the MPV.5.The PLR in the group B and group C were both higher than that in the group A,which was statistically significant(P<0.05);the PLR was negatively correlated with MPV and LYM;the PLR was negatively correlated with PLT;the area under the ROC curve for PLR was 0.631.which was statistically significant(P<0.05),the sensitivity、specificity and cut-off value of PLR were 44.8%、82.5%、134.33.Conclusion:1.The PLR、NEU of the pregnant women could be used as the new reference index to predict and diagnose GDM.2.The PLT、LYM、MPV of the pregnant women has no significant reference value in predicting and diagnosing GDM.
