| Objective:Treating patients with distal radius fracture of type C by simple metacarpal plate internal fixation,metacarpal plate internal fixation with external fixator,simple external fixator and measuring the length of distal radius in three groups during 10 to 12 months after operation,to explore which fixation is the most beneficial to the length of distal radius.By measuring and comparing the wrist motion and wrist score in 10 or 12 months after operation,to explore which fixation is the most beneficial to the wrist function recovery.Methods:from September 2016 to September 2018 in Dalian Friendship Hospital,48 cases of orthopaedic inpatients(all of whom were classified as AO type C)were selected.16 cases were treated with simple internal fixation(group A)and 16 cases with internal fixation and external fixator frame(group B),16 cases were treated with external fixator alone(group C).There were 26 males and 22 females,The minimum age was 30 and the maximum age was 65,the average age was(46.56±10.93).The causes of injury were 6 cases of traffic accident and 42 cases of falling injury.Classification:26 cases in type C1,14 cases in type C2,and 8 cases in type C3.There were 7 males and 9 females in group A with mean age of(45.25 ± 10.09)years,3 cases with car accident injury,13 cases with falling injury,9 cases with C1 type,5 cases with C2 type,2 cases with C3 type.10 males and 6 females in group B,age 31-65 years,mean age(47.56±11.10)years,2 cases with car accident injury,14 cases with fall injury,9 cases with type C1,4 cases with type C2,3 cases with type C3.C group consists of 9 male cases and 7 female cases,age 32-65years,mean age(46.88±11.)years.1 case of car accident injury,15 cases of falling injury,8 cases of C1 type,5 cases of C2 type and 3 cases of C3 type.All patients were followed up for 10-12 months(mean 11.0210.53months).SPSS25.0 is used to deal with the related data.Standard deviation of mean is used in all metrology,difference analysis is used in metrological data,frequency and percentage are used for all counting,and X2 test is used for counting data.Results:There was no significant difference between the three groups of data in age,sex,injury and fracture(p>0.05).All three groups were followed up in all three groups,and all of them were healed.In group A,there were one case of traumatic arthritis.There were two cas61es of ulnar impact syndrome in group C,and the rest of the wrist joint function was restored without complication.The mean height of the distal length of the three groups was(3.13±0.74)mm,(1.88±0.40)mm,(4.62±1.47)mm,p<0.05,and the difference in the length of the distal radial of the three groups was statistically significant.In three groups,the angle of ulnar deviation after operation in 12 months was(20.13±1.20)°,(21.56±1.41)°,(19.44±1.15)°,(10.63±0.96)°,(11.50±0.97)°,(9.88±1.02)°,P<0.05,respectively.There were statistically significant differences in the angle of palmar inclination and ulnar deviation between the three groups on imaging.The carpal function score in group A was excellent in 9 cases,good in 4 cases,moderate good in 2 cases,poor in 1 case,excellent in B group in 11 cases,good in 3 cases,moderate good in 0 case,and excellent in group C in 8 cases,good in 2 cases,moderate good in 3 cases,poor in 3 cases,P<0.05,the difference was significant(P<0.05).There is statistical significance.Conclusion:the simple internal fixation of palmar lateral plate,the internal fixation with external fixation and the external fixation are effective in the treatment of distal radius fracture of type C.But the the internal fixation with external fixation can maintain the length of distal radius better and recover the function of wrist joint better.It is a better choice for the treatment of complex distal radius fracture. |
