OBJECTIVE: To observe the clinical effect of acupuncture combined with Xiaoyao Siwu Decoction on ischemic optic neuropathy.METHODS: Forty patients(49 eyes)with ischemic optic neuropathy who met the inclusion criteria were randomly divided into the experimental group and the control group using the parallel randomized control method.The control group was treated with compound anisodine injection 0.2mg subcutaneously beside the superficial temporal artery on the affected eye side once a day.The experimental group was additionally treated with acupuncture and Xiaoyao Siwu Decoction on the basis of the control group.A course of treatment is given every 14 days for 3 courses of treatment,with one week interval between each course of treatment.The changes of TCM syndrome score and objective index(vision,visual field OCT FVEP)before and after treatment were observed and statistically analyzed.RESULTS: The final number of observed cases was 40(49 eyes),20(26 eyes)in the experimental group and 20(23 eyes)in the control group.(1)Baseline data analysis: there was no difference in objective indicators of TCM syndrome score between gender,age,disease course and eye group(P>0.05).(2)Efficacy of TCM syndromes: The total effective rate of the experimental group is 75%,and that of the control group is 35%.The experimental group is superior to the control group in terms of obvious efficiency and effective rate.After treatment,the comparison between the two groups showed statistical significance in headache,eye pain,mental depression,thoracic rib pain,dry mouth and bitter taste(P<0.05).The experimental group is better than the control group.(3)Objective indicators: After treatment,the vision and visual field in theexperimental group improved(P<0.01),but there was no significant difference in the control group(P>0.05).After treatment,the latency of P wave was shortened and the amplitude was increased in both groups(P<0.05).After treatment,the RNFL value in the experimental group was lower than that before treatment,but the difference was not statistically significant(P > 0.05).The RNFL value of the control group was significantly lower than that of the experimental group,with statistically significant differences(P<0.01).Conclusion: 1.Acupuncture combined with Xiaoyao Siwu Decoction has a significant effect on non-arteritis anterior ischemic optic neuropathy,which is better than simple subcutaneous injection of compound anisodine injection beside superficial temporal artery.2.Acupuncture combined with Xiaoyao Siwu Decoction is more effective in improving vision,reducing visual field defect,increasing P wave amplitude,shortening incubation period,relieving headache,eye pain,mental depression,chest pain,xerostomia and bitter taste than subcutaneous injection of compound anisodine injection by superficial temporal artery alone.3.Acupuncture combined with Xiaoyao Siwu Decoction has good safety,convenient operation and low cost in the treatment of non-arteritis anterior ischemic optic neuropathy.It has the characteristics and advantages of " combination of acupuncture and medicine" in Chinese medicine and has high clinical practical value.
