| Objectives:To study the clinical effect and application of cytoreductive surgery combined with intraperitoneal hyperthermia in patients with peritoneal metastasis of gastrointestinal cancer.Methods:A retrospective study of 72 patients with peritoneal metastasis of gastrointestinal cancer admitted to our hospital from 2014 to 2015.They were divided into observation group(n = 33 cases)and control group(n = 39 cases)according to the actual treatment methods.the 33 patients of observation group were treated with cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy.While The 39 patients of control group performed only cytoreductive surgery.Both groups underwent intravenous chemotherapy with chemical drugs after surgery.Statistics and analysis on the related curative effect of two groups of patients.Results:Comparison of age,weight,sex and type of tumor between the two groups of patients.no statistical significance of the difference(P>0.05).Comparison of perioperative complications,no statistical significance of the difference(P>0.05),Preoperative comparison of positive detection rate of intraperitoneal free cancer cells,the observation group was 63.6%,control group was 66.7%,no statistical significance of the difference(P>0.05).Comparison of detection rate of free cancer cells in abdominal cavity after operation,the observation group was18.2%.control group was 41.0 %,the observation group was significantly lower than the control group,and the difference was statistically significant(P <0.05).Comparison of CEA,before surgery,observation group was(185.5±26.9)Ng/ml,control group was(180.0±19.6)Ng/ml.After the surgery,the observation group was(140.3±14.6)Ng/ml,control group was(160.0±17.0)Ng/ml,the difference was statistically significant(P<0.05).The 1-year and 2-year survival rates of the observation group were:90.9% and 57.6 %,the rates of local recurrence and metastasis in 1-year and 2-y-ear were 21.2% and 36.4%,the 1-year and 2-year survival rates were71.8%and 33.3% in the control group,The rates of local recurrence and metastasis in1-year and 2-year were 46.2% and 61.5%,It showed that the survival rate of 1-year and 2-years was significantly higher than that of the control group.The rate of local recurrence and metastasis in the observation group was lower than that in the control group in the 1-year and the 2-year,the difference was statisticallysignificant(P<0.05).Comparison of 3-year survival rate,local recurrence and metastasis,there was no significant difference between the two groups(P>0.05).Conclusions:Patients with peritoneal metastasis of gastrointestinal cancer,application of cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy,it can significantly reduce the detection rate of abdominal free cancer cells,significantly reduce the level of serum tumor markers,increase the survival rate of patients,reduce the local recurrence and metastasis rate,improve theprognosis. |
