| Objective:The petroclival tumors refer to tumors originating from the medial petrous bone of the medial part of the CNV-_Ⅷ cranial nerve outflow on the middle and upper slope centered on the petroclival fissure,and also include tumors originating from petroclival apex,Meckel cavity and posterior clinoid process.Due to the lack of systematic comparative study on the advantages and disadvantages of various surgical approaches in petroclival region,how to choose the surgical approach scientifically and reasonably combined with specific conditions in clinical practice has become a difficult problem.Compare and analyze the clinical efficacy of different surgical approaches for resection of petroclival tumors,so as to select a reasonable surgical approach based on clinical practice and the growth characteristics of petroclival tumors.Material and Method:From January 2008 to December 2018,26 patients with petroclival tumors in Shandong Provincial Hospital were selected as the study subjects.All cases were confirmed by surgery and pathology.There were 7 males and 19 females,aged 24-78 years,with an average age of 54.88 ± 12.65.The hospitalization time was 18.68(+9.63)..The clinical manifestations included dizziness,headache,tinnitus,hearing loss in 6 cases,facial numbness in 6 cases,gait instability in 1 case,and visual impairment in 4 cases.There were 1 case of limb movement and sensory disturbance,3 cases of drinking cough and 3 cases of physical examination.According to the patient’s condition,different surgical approaches were adopted.The clinical manifestations,characteristics,surgical approaches,treatment results and postoperative clinical complications were compared and analyzed.Results:Among the 26 cases,16 were totally resected(GTR),6 were subtotally resected(STR),and 4 were mostly resected.Through the comparative analysis and statistical analysis of the degree and effect of tumor resection through different surgical approaches,the following results were obtained:the focus of surgical treatment is to give appropriate surgical approaches to different types of tumors,which plays an important role in improving the resection rate of tumors,the incidence of complications and the recovery of body function.The choice of surgical approach is closely related to the location,growth pattern,size and invasive extent of tumors.Each surgical approach has its own advantages and disadvantages.Conclusions:Selection of appropriate surgical approaches for petroclival tumors in different locations can improve the resection rate of tumors,reduce the occurrence of complications,and effectively improve the recovery of patients after surgery.The principle of choosing surgical approach is not only to maximize the exposure and removal of tumors,but also to ensure the minimal damage to surrounding tissues and blood vessels and nerves,so as to minimize the damage to the patient’s body. |
