| Objective To observe the function of caffeine citrate in the treatment of apnea of prematurity on cerebral cortical activity by amplitude-integrated electroencephalogram.Methods A total of 56 case of premature infants with primary apnea of prematurity who were hospitalized from January 1,2016 to January 1,2017,whose gestational age ranged from 27 weeks to 34 weeks.Among them,28 cases of premature infants with primary Apnea of Prematurity were treated with caffeine citrate as treatment group,28 cases of non-apnea preterm infants as control group,and according to gestational age were divided into 16 cases in 27-32week(27 week<gestational age<32 week)group,12 cases in 32-34 week(32 week≤gestational age<34 week)group.The treatment group on the first day of medication(2 hours before treatment)and the third day of medication,the seventh day of medication,the second day after the medication were monitored by cerebral function analyzing monitor(NicoletOne)with 4-6 hours.Control group on the first day after birth,the third day,the seventh day were monitored by cerebral function analyzing monitor(NicoletOne)with 4-6 hours.The changes of lower margin amplitude and bandwidth of amplitude-integrated electroencephalography were observed dynamically on the first day(2 hours before treatment),on the third day of medication,on the seventh day of medication,on the second day after the medication,and the deviations of the lower margin amplitude and bandwidth on the first day and the third day,the deviations of lower bound voltage value and bandwidth on the third day and the seventh day.Results The lower margin amplitude and bandwidth of amplitude-integrated electroencephalography on the first day of medication(2 hours before treatment),the third day of medication,the seventh day of medication and the second day after the medication were statistically significant difference(P<0.05)between groups in 27-32week group.The differences of the lower margin amplitude and the bandwidth of the seventh day of medication and the second day after the medication was no statistical difference(P>0.05)in 27-32week group.The lower margin amplitude and bandwidth of amplitude-integrated electroencephalography on the first day of medication(2 hours before treatment),the third day of medication,the seventh day of medication and the second day after the medication were statistically significant difference(P<0.05)between groups in 32-34 week group.The differences of the lower margin amplitude and the bandwidth of the seventh day of medication and the second day after the medication was no statistical difference(P>0.05)in 32-34 week group.There was a statistically significant difference in deviations of the lower margin amplitude and bandwidth difference between the treatment group and the control group on the first day and the third day in 27-32week group(P<0.05).There was a statistically significant difference in deviations of the lower margin amplitude and bandwidth difference between the treatment group and the control group on the third day and the seventh day in 27-32week group(P<0.05).There was a statistically significant difference in deviations of the lower margin amplitude and bandwidth difference between the treatment group and the control group on the first day and the third day in 32-34 week group(P<0.05).There was a statistically significant difference in deviations of the lower margin amplitude and bandwidth difference between the treatment group and the control group on the third day and the seventh day in 32-34 week group(P<0.05).Conclusion lower margin amplitude and bandwidth of amplitude-integrated electroencephalography can reflect the maturity of brain function of premature infant.Caffeine for the treatment of apnea of prematurity excite the respiratory center,at the same time,promote the maturity of brain function of premature infant,in some extent. |
