| [Objective]To study the detection of colorectal polyp,the detection rate of adenoma,the related factors that affect the detection rate of adenoma can guide clinical endoscopy physician in order to improve the adenoma detection rate and to improve the quality of electronic colonoscopy.[Methods]All patients who underwent colonoscopy in Weihai Municipal Hospital from September 1,2016 to December 31,2016,a total of 883 patients are recorded.29 patients are excluded from those who do not reach ileocecum,and 854 patients are included in statistical analysis.393 patients are male and 461 patients are female.The sample data according with study conditions are analyzed,and the number of colorectal polyps is detected,and the detection rate of colorectal polyps is calculated.The number of adenomatous polyps is estimated by individual adenoma-to-polyp-detection rate ratio,the detection rate of adenomas is calculated.Related factors such as sex,age,body mass index,anaesthesia,operation mode,insertion time,withdrawal time,and intestinal preparation are recorded respectively.A single factor chi 2 test is used to analyze the relationship between the adenoma detection rate and the factors mentioned above.The above factors are introduced into the two category Logistic regression analysis,and the mutual interference between the above factors is excluded.The independent risk factors affecting the adenoma detection rate are analyzed.[Results]1.Of the 854 samples,polyps are detected in 351 cases,and the numbers of polyps and adenomatous polyps are 525 and 225,respectively.The total detection rate of adenoma is 26.35%.2.Theχ2 test shows:the adenoma detection rate and gender(χ2=10.166,P<0.05),age(male χ2=23.105,P<0.05;femaleχ2=11.366,P<0.05),operation mode(χ2=9.278,P<0.05),withdrawal time(χ2=67,293,P<0.05).The method of anesthesia(χ2=0.027,P>0.05)and insertion time(χ=11.198,P>0.05)do not affect the adenoma detection rate.The effect of body mass index and intestinal preparation on the adenoma detection rate are not significant,but there are still a certain correlation with the adenoma detection rate.3.Logistic regression analysis shows:that gender(OR:3.819;95%CI:1.076-13.547,P<0.05),age(OR:5.092;95%CI:2.766-9.377,P<0.05),BMI(OR:0.048;95%CI:0.018-0.131,P<0.05),operation-mode(OR:3.199;95%CI:1.210-8.458,P<0.05),insertion-time(OR:0.019;95%CI:0.009-0.044,P<0.05),withdrawal time(OR:686.602;95%CI:202.700-2325.711,P<0.05),intestinal preparation(OR:0.117;95%CI:0.032-0.428,P<0.05)are independent risk factors influencing the adenoma detection rate,anaesthesia do not find a correlation exists and the adenoma detection rate.[Conclusion]1.The detection rate of male adenoma is higher than that of female,and the detection rate of adenoma in the aged group and middle age group is higher than that of the young group.2.Compared with the double colonoscopy,a single colonoscopy has a higher adenoma detection rate.3.Insertion time can influence the adenoma detection rate.When the time of withdrawal is longer than 6 minutes,the detection rate of adenoma is significantly increased,and it increases with the extension of withdrawal time.4.Body mass index(BMI)is associated with the adenoma detection rate.5.The cleanliness of bowel preparation is positively correlated with the adenoma detection rate.Adequate bowel preparation can improve the adenoma detection rate.The highest degree of bowel preparation could not achieve the highest adenoma detection rate.6.The method of anaesthesia do not affect the detection rate of adenoma.7.The sex,age,body mass index,operation mode,insertion time,withdrawal timeand intestinal cleanliness are the independent risk factors that affect the adenoma detection rate. |
