BackgroundWith the introduction of "precision treatment",the treatment of lung cancer has entered the stage of "individualized treatment".As a targeted treatment that can significantly prolong the survival time of patients with advanced lung cancer,it has become an indispensable and important means in the treatment of non-small cell lung cancer(NSCLC).Epidermal growth factor receptor tyrosine kinase inhibitor(EGFR-TKI)has the side effect is small,taking convenience and good patients’ compliance and obvious effects,has been widely used in clinical,but its better curative effect is confined to patients with positive target genes,so before taking targeted drugs,EGFR gene detection is a necessary condition.According to clinical experience and previous research results,there is a certain relationship between EGFR mutation status and clinical pathological characteristics of patients.This study mainly explored the relationship between clinical characteristics,pathological subtypes and EGFR gene mutation in patients with lung adenocarcinoma,so as to provide a basis for clinicians to better predict the EGFR gene mutation status of patients.ObjectiveTo summarize the clinical records of patients with lung adenocarcinoma diagnosed in a single center of the first affiliated hospital of zhengzhou university and EGFR gene detection within 1 year after surgery,to explore the relationship between EGFR gene mutation and lung adenocarcinoma patients with clinical features and pathological subtypes,and provide a basis for better prediction of EGFR gene mutation status in patients.MethodsData of patients diagnosed with lung adenocarcinoma and diagnosed with EGFR gene testing after surgical resection in the department of thoracic surgery of the first affiliated hospital of zhengzhou university from June 2016 to June 2017 were retrospectively analyzed,and a total of 269 patients were effectively included.The correlation between EGFR mutation status and lung adenocarcinoma was determined by comparing the differences between the mutant group and the wild group in gender,age,smoking history,history of hypertension,history of diabetes,blood type and pathological subtypes.ResultsEGFR gene mutations in 269 patients with lung adenocarcinoma,165 cases(61.3%),including 18 exons mutation in 2 cases,19 exons mutation in 72 cases,20 mutations exon 8 cases,21 exon mutation in 80 cases,18 and 20 exon double mutation in 1 case,19,20 exon double mutation in 1 case,20,21 exon double mutation in 1 case,104 cases of wild type.Among them,133 were males and 136 were females.The average age was 59.42 years old.And smoking patients 77 cases,192 patients with no smoking,high blood pressure patients 70 cases,199 cases of patients with high blood pressure,diabetes patients,24 cases,245 cases of patients with diabetes and 65 patients with type A,type B blood,91 cases of patients,28 patients with AB blood to type O blood of 85 cases patients with infiltrating type 220 cases of adenocarcinoma,mucous adenocarcinoma type 21 cases,adenocarcinoma,3 cases of intestinal type,glands squamous carcinoma 4 cases,21 cases of other uncertain pathological subtype.Single factor correlation analysis showed that there were statistically significant differences in gender,smoking history and pathological subtypes between the mutant group and the wild group(P<0.05).There was no significant difference in age,history of hypertension,history of diabetes and blood type between the two groups(P>0.05).Multivariate regression analysis showed that gender pathological subtypes were independent risk factors for EGFR mutation in patients with lung adenocarcinoma.ConclusionSingle factor correlation analysis showed that EGFR mutation in lung adenocarcinoma was more common in female,non-smoking,invasive and adenosquamous carcinoma patients.There was no significant correlation between the status of gene mutation and patients’ age,history of hypertension,history of diabetes,etc.The relationship between ABO blood group and EGFR gene mutation status remains to be further explored.Multivariate analysis showed that gender and pathological subtypes were independent risk factors for EGFR mutation in patients with lung adenocarcinoma: women,invasive adenocarcinoma and adenosquamous carcinoma had higher mutation rates,while men,mucinous adenocarcinoma and intestinal adenocarcinoma had lower mutation rates.
