| Objective: Exploring the economic value of integrated Chinese and Western medicine treatment for acute cerebral infarction. Methods:Included from January 2018 to December 2018,238 patients with acute ischemic stroke in the Department of Neurology,First Affiliated Hospital of Hunan University of Traditional Chinese Medicine, divided into 8 groups according to the treatment method, group a.Western medicine routine treatment + Chinese medicine injection treatment,Group b.Western medicine routine treatment + Chinese medicine injection treatment + Chinese herbal medicine decoction treatment, group c.Western medicine routine treatment + Chinese medicine injection treatment + acupuncture treatment, group d.Western medicine routine treatment + Chinese medicine injection treatment + Chinese medicine characteristic treatment, group e.Western medicine Conventional treatment + traditional Chinese medicine injection + Chinese herbal decoction + acupuncture treatment, group f.Western medicine routine treatment +Chinese medicine injection treatment + Chinese herbal medicine decoction +traditional Chinese medicine treatment, group g.Western medicine routine treatment+ Chinese medicine injection treatment + acupuncture treatment + Chinese medicine Characteristic treatment, group h.Western medicine routine treatment + Chinese medicine injection treatment + Chinese herbal medicine decoction + acupuncture treatment + traditional Chinese medicine treatment, using the National Institutes of Health Stroke Scale(NIHSS)for the evaluation of the efficacy of each group of patients before and after treatment, through the hospital The medical records were used to find out the direct medical expenses of patients, and the cost-effectiveness ratio(CER)and incremental cost-effectiveness ratio(ICER)were used for health economic evaluation. Results:There were no significant differences in the pre-treatment general data and IHSS scores between the groups(P>0.05).After treatment with integrated Chinese and Western medicine, the difference of NIHSS scores in each group was statistically significant(P<0.05),f group(Western medicine routine treatment + traditional Chinese medicine injection + Chinese herbal decoction + traditional Chinese medicine treatment)and h group(Western medicine routine treatment + Chinese medicine)The injection treatment + Chinese herbal decoction + acupuncture treatment + traditional Chinese medicine treatment)NIHSS scores were significantly different, respectively 2.87 ± 1.26,2.97 ± 0.84;a group(Western medicine routine treatment + Chinese medicine injection treatment)and f group hospitalization days are shorter,10.73 days,10.93 days respectively;the three groups with lower total direct medical expenses are: the average total cost of group a is 15821.39 yuan, and the average total cost of group d(Western medicine routine treatment + traditional Chinese medicine injection treatment + traditional Chinese medicine treatment)is16726.36 yuan,f The average total cost of the group is 17102.71 yuan;the lowest CER is the f group, and the NIHSS is reduced by 1 point, which costs 5966.06 yuan. The lowest ICER ratio is the f group(1525.38 yuan),indicating that each increase is more. One effect, the f group needs to increase the minimum cost. Conclusion:The combination of traditional Chinese medicine and western medicine has a good curative effect on acute cerebral infarction. This study has obtained the traditional Chinese medicine treatment + traditional Chinese medicine injection +Chinese herbal medicine decoction + traditional Chinese medicine treatment program for the treatment of good and low cost Chinese and Western combination treatment, which is worthy of clinical application. |
