Objective: To compare The changes of high order aberration of cornea after SMILE and FS-LASIK for high myopia in the early stage. Methods: In this prospective non-randomized case-controlled study,High myopia patients who received myopia correction in laser myopia correction center of the first affiliated hospital of guangxi medical university were collected and divided into 25 cases(48 eyes)in SMILE group and 20 cases(38 eyes)in FS-LASIK group according to their surgical methodsl.Routine ophthalmic examinations were performed. The patients were followed 1 months to monitor uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),refractive error(includes spherical cylindrical and equivalent spherical),intraocular pressure,Pentacam measured corneal high order aberration in both eyes. The results of UCVA,higher order aberrations of cornea and subjective visual quality at 1 month after surgery were observed.Independent sample t test or a single factor ANOVA were used to verify the normal distribution of data, and non-parameter mann-whitney U test and Wilcoxon rank sum test was used for statistical analysis in non-normaldistribution.Results:1.The efficacy indexes of the FS-LASIK group and SMILE group were 0.91 ± 0.16 and 0.92 ± 0.11,and there was no significant difference between the UCVA groups after surgery(P>0.05).UCVA in SMILE group was improved between 1 day and 1 week and between 1 week and 1 month after surgery(z=-2.297,P=0.022<0.05;z=-2.995,P=0.003<0.05).There was no significant difference in UCVA after FS-LASIK(P>0.05).2.The total high-order aberration RMS,oblique clover aberration and fourth-order spherical aberration in the FS-LASIK group increased significantly 1 month after surgery compared with that before surgery(P < 0.05),while the other variables at the time points of follow-up showed no statistical significance(P>0.05).3.The total high-order aberration, vertical coma aberration and fourth-order spherical aberrationin the SMILE group increased significantly 1 month after surgery compared with that before surgery(P < 0.05),the changes of total high-order aberration in one day and one week were increased significantly(P<0.05);4.Compared between the two groups,One month after surgery, the total high-order aberration RMS and fourth-order spherical aberration in the FS-LASIK group were significantly increased, and the vertical coma in the SMILE group was significantly increased compared with that in the FS-LASIK group,(t=-2.108,P<0.05;t=-3.344,P=0.001<0.05;t=-5.793,P<0.05).The other were not statistically significant(P>0.05).5.There was no significant difference in ocular symptoms and subjective visual quality between the two groups 1 month after the operation(P>0.05).Conclusion: 1.Both SMILE and FS-LASIK were performed for high myopia, and the high order aberration of cornea was changed to some extent after the surgery,The total high order aberration and spherical aberration ofFS-LASIK changed more than SMILE,The Evertical coma of SMILE changed more than FS-LASIK.2.In the early postoperative period,FS-LASIK was more stable than SMILE.3.Both SMILE and FS-LASIK achieved satisfactory postoperative results subjectively.
