Object:This paper aims to explore the application value of high-resolution magnetic resonance in vulnerable plaque through its function of imaging identification on carotid plaque,so as to provide early warning for secondary prevention of stroke.Materials and Methods:In the period from March 2018 to March 2019,neurological outpatients and in-hospital patients in our hospital?China-Japan Union Hospital of Jilin University?were randomly divided into symptomatic group?group A?and non-symptomatic group?group B?according to the occurrence of acute ischemic cerebrovascular disease.According to the stenosis degree of carotid ultrasound vessels,they were further divided into mild stenosis group A1?stenosis degree<50%?,moderate stenosis group A2?the degree of stenosis ranged from 50%to 69%?and A3 group with severe stenosis?stenosis degree?70%?were enrolled in 12 patients in each group.The non-symptomatic group was enrolled in the same standard as symptomatic group.A total of 72 patients were enrolled in the study.High Resolution Magnetic Resonance Imaging?HR-MRI?was used in the inspection on carotid plaques in each group,and then the plaque images collected by HR-MRI in each group were collected.The improved MR AHA was used to classify the plaque,and the relationship between symptomatic group and asymptomatic group as well as the nature of plaque was analyzed and compared.The relationship between the degree of carotid stenosis and the nature of carotid plaque was also analyzed and compared.The advantages of HR-MRI in vulnerable plaque recognition were discussed by comparing with ultrasound.Results:?1?There were 27 cases with vulnerable plaque in the symptomatic group and17 cases with vulnerable plaque in the asymptomatic group.By comparison,the proportion of patients with vulnerable plaque in the symptomatic group was higher than that in the asymptomatic group,with statistical difference?P<0.05?.?2?There were 8 cases with vulnerable plaque in mild stenosis group,15 cases with vulnerable plaque in moderate stenosis group,21 cases with vulnerable plaque in severe stenosis group,and the proportion of vulnerable plaque in three groups with different stenosis degree brings statistical difference?P<0.05?.The more severe the stenosis degree,the higher the proportion of vulnerable plaque?P<0.05?.?3?Comparing the imaging characteristics of carotid plaque in carotid ultrasound and carotid high-resolution magnetic resonance imaging,it shows that carotid ultrasound can judge the nature of plaque by different echoes of plaque,and carotid high-resolution magnetic resonance can identify the internal different components of plaque and quantify its nature.Conclusion:?1?The proportion of patients with carotid vulnerable plaques in symptomatic group was higher than that in non-symptomatic group.?2?The proportion of patients with vulnerable plaque:severe stenosis group>moderate stenosis group>mild stenosis group.?3?HR-MRI of carotid can distinguish and quantify different components of plaque,which has obvious advantages in identifying vulnerable plaque.
