| Objective:Evaluate the difference of the therapeutic effect of contact dermatitis’ differentdegree induced by acupoint application on the recurrence of asthma,and to explore the potential feasibility of Raman spectroscopy in distinguishing the degree of dermatitis.Methods:58 asthma patients in remission stage were selected from the acupuncture rehabilitation department of affiliated Hospital of Nanjing University of traditional Chinese Medicine in the summer of 2018.(1)Asthma quality of life questionnaire scores were recorded before treatment,at the end of treatment,3 months after treatment and 6 months after treatment.(2)According to the ’skin irritation and sensitization response score’,within 24 hours after the last acupoint application,the patients with dermatitis induced by acupoint application was divided into four groups:dermatitis-free group,mild contact dermatitis(abbreviated as mild group),moderate contact dermatitis(abbreviated as moderate group)and severe contact dermatitis(abbreviated as severe group),among which 24 cases were mild group,16 cases in moderate group,6 cases in severe group and 12cases dermatitis-free group.(3)The Raman data of the right Feishu were obtained by Raman spectrometer before treatment and within 24 hours after the last application.The average Raman spectra and two-dimensional and three-dimensional distribution images of different components were obtained by data processing.According to the two-dimensional images of different components generated by Raman data,the skin reaction after acupoint application was also divided into different groups.(4)In order to compare the coincidence between the two methods of the dermatitis degree’s grading,classification under ’skin irritation and sensitization response score’is presented in the two-dimensional and three-dimensional distribution images in the form of color recognition;while classification under ’Raman spectrum’is presented in the two-dimensional and three-dimensional distribution images in the form of separation trend.By comparing the dermatitis degree’s grading according to color recognition and and separation trend in the two-dimensional and three-dimensional distribution images,taking the"skin irritation and sensitizing response score standard" as the control,the coincidence of the two methods was observed,exploring the potential feasibility of Raman spectroscopy in distinguishing the degree of dermatitis.Results:(1)①There was statistical difference between before and after treatment in 58 asthma patients(P<0.01),among which there were both statistical difference between before and after treatment in mild and moderate group(P<0.05),besides there were neither statistical difference between before and after treatment in severe group and non-skin inflammation group(P>0.05).?Tests of between-subjects effect:Group P<0.05,there was statistical difference in the total score of AQLQ between different groups,among which there was statistical difference of AQLQ score at each time point after treatment between mild group and moderate group(P<0.05);there was statistical difference of AQLQ score at each time point after treatment between severe group and moderate group(P<0.05);there was statistical difference between mild group and non-dermatitis group at the end of treatment(P<0.05);there was statistical difference between severe group and non-dermatitis group at the end of treatment(P<0.05);there was no statistical difference between mild group and non-dermatitis group at 3 months and 6 monafter treatment(P>0.05);there was no statistical difference between severe group and non-dermatitis group at 3 ITmonths and 6 months after treatment(P>0.05).(2)①Before treatment,the average Raman spectrumhas 13 characteristic peak,which positioned at 266cm-1,335cm-1,413cm-1 531cm-1,639cm-1,686cm-1,846cm-1,869cm-1,986cm-1,1000cm-1,1344cm-1,1535cm-1 and 1776cm-1band.After treatment,the average Raman spectrumhas 13 characteristic peak,which positioned at 266cm-1,335cm-1,415cm-1,531cm-1,639cm-1,688cm-1,846cm-1,869cm-1,986cm-1,1004cm-1,1344cm-1,1535cm-1 and 1776cm-1 band.Thee was no significant difference in the peak position of the average Raman spectrum before and after treatment.while the main differences are the peak intensity of characteristic peaks.There was no significant difference in the characteristic peak of contact dermatitis induced by acupoint application,and the positions of the characteristic peak were at 266cm-1,335cm-1,415cm-1,531cm-1,639cm-1,688cm-1,846cm-1,869cm-1,986cm-1,1004cm-1,1344cm-1,1535cm-1 and 1776cm-1 band,while the main differences are the peak intensity of characteristic peaks,among which the average Raman spectra of the moderate group were significantly different from the other three groups.(2)The Raman data of 58 patients with asthma before and after treatment(including mild group,moderate group and severe group)showed discrete trend after principal component analysis(model parameter R2X=0.968,Q2=0.957).The Raman data of different degrees of contact dermatitis induced by acupoint application also showed discrete trend after principal component analysis(model parameter R2X=0.98,Q2=0.969),among which the individual difference of the severe group and moderate group was great,featured as distributed discrete;the aggregation of mild group and non-skin inflammation group was better.Two patients were included in the severe group under’skin irritation and sensitization response score’,while it’s corresponding Raman data were statistically processed and then included in the moderate group.Three cases were included in the moderate group under ’skin irritation and sensitization response score’,while it’s corresponding Raman data were statistically processed and included in the mild group.Four cases were included in the non-skin inflammation group under ’skin irritation and sensitization response score’,while it’s corresponding Raman data were statistically processed and included in the mild group.Conclusion:(1)The curative effect of acupoint application on asthma recurrence is definited.It is different when inducing different degrees contact dermatitis,among which the effect of mild and moderate dermatitis is better than that of non-dermatitis and severe dermatitis.(2)There were differences in Raman spectra of different degrees of dermatitis,and the average Raman spectrum of moderate group was significantly different from the other groups.There was a certain coincidence between the two methods,but because of the severe group samples is small and the large individual differences,this study only shows that Raman spectroscopy can distinguish the moderate group from the non-dermatitis group and the mild group. |
