TCM Thread-dragging Therapy Combined With Autologous Adipose-derived Stem Cells Transplantation In The Treatment Of Anal Fistulas In Crohn’s Disease(CD)

Objectives:To observe the clinical efficacy of TCM thread-dragging therapy combined with autologous adipose-derived stem cells transplantation in the treatment of anal fistulas in Crohn’s Disease(CD),and to preliminarily explore the safety and effectiveness of autologous adipose-derived stem cells in the treatment of anal fistula in CD.Methods:Patients with CD anal fistula who met the inclusion criteria were randomly divided into Treatment Group(13 cases)and Control Group(17 cases).Control Group:treat CD anal fistula with TCM thread-dragging therapy;Treatment Group:after the treatment of TCMthread-dragging therapy for 2 weeks,make injection of autologous adipose-derived stem cells for 2×109 locally.The main symptoms,CRP,ESR,fecal calprotectin,albumin,platelet,IBDQ,CDAI,PDAI and MRI were observed,recorded and compared before and after treatment,and the changes of the disease were evaluated.Results:29 cases completed the experiment,where there was 1 expulsion(16/17)case in Control Group.According to the evaluation of closure criteria for CD anal fistula in WGO’s C.lassification,diagnosis and multidisciplinary consensus of anal fistula in Crohn’s Disease in 2104:the effective rates of the Control Group and the Experimental Group were 100%,and the clinical closure rates were 53.8%(7/13)and 31.2%(5/16)respectively;there were significant differences in CRP,PDAI and IBDQ scores between the patients of two groups(P<0.05);there was no significant difference in clinical symptoms such as abdominal pain,diarrhea,fecal calprotectin,ESR,albumin and platelet test between the patients of two groups.Conclusion:(1)the treatment of anal fistulas in Crohn’s Disease with TCM thread-dragging therapy has unique advantages and certain curative effect;(2)the treatment of anal fistulas in Crohn’s Disease with TCM thread-dragging therapy combined with autologous adipose-derived stem cells transplantation has higher fistula closure rate.One year after operation,the effective rate was 84.6%;(3)Compared with the long-term effect,two weeks after operation,the living quality of the patients in Experimental Group was significantly improved.Some patients’ fistulas have healed,and the short-term effect was also worth affirming;(4)ADSC can play a certain role in controlling inflammation.
