BackgroundsThe clinical outcome of Hepatitis B virus(HBV)infection is diverse and is closely related with various factors such as viruses,host immunity,and host gene susceptibility.Ninety percent of people infected with HBV during the perinatal period will become chronic hepatitis B virus infection,while less than 30 percent of people infected with the virus during infancy will progress to chronic,with age increasing.People aged 5 and above are less likely to develop slowly,with less than 10 % chance.HBV infection in the human body will develop into the following five different diseases: acute HBV infection recovery period,asymptomatic HBV carriers,chronic hepatitis B viral hepatitis B(CHB),cirrhosis(LC)Even further progressed to Hepatocellular carcinoma(HCC).Previous studies have shown that the different outcomes of HBV infection in humans are caused on the one hand by the effects of the virus itself,and on the other hand by the interaction of various factors,including the following: environment,genetic diversity,host immune response,etc..The number of copies of the host gene(CNVs)is a new form of gene variation discovered in recent years,which can cause changes in the copy number of the host gene.CNVs can affect gene expression and further affect disease susceptibility and disease progression through the gene sequence location changes.This study mainly analyzes the difference in the frequency of the number of copies of the APOBEC3 B gene at different stages after HBV infection,so as to further explore the influence factors of the outcome after HBV infection.It is expected that this study will provide theoretical support for elucidating the different transitions after HBV infection.Aims To investigate the difference of frequency distribution of APOBEC3 B gene copy number in patients with HBV infection and its role in clinical outcome.Methods Peripheral blood samples were collected from 296 patients with acute self-limited recovery(AHB)after HBV infection and 819 patients with chronic HBV at different stages of infection.Chronic HBV infection group included 444 patients with chronic hepatitis B(CHB),252 patients with liver cirrhosis(LC)and 123 patients with hepatocellular carcinoma(HCC).After DNA extraction and purification,Accucopy method was further adopted to detect the distribution frequency of APOBEC3 B gene copy number in peripheral blood of the two groups,and relevant clinical data of the subjects were collected.SPSS software was used to conduct statistical analysis on copy number distribution of patients in different groups,so as to further analyze the correlation between APOBEC3 B copy number distribution difference and chronic disease and disease progression after HBV infection.P < 0.05 indicates a statistical difference.Results1.In the outcome of HBV infection,the proportion of APOBEC3B<2 copy number in acute self-limiting HBV infection group is significantly lower than the proportion in chronic HBV infection group,46.96% vs 57.95%,P=0.001.2.In the disease progression of chronic HBV infection,the proportion of APOBEC3B<2 copy number in the CHB,LC and HCC groups gradually increases,11.03%,13.94%,22.76%,P<0.01.3.There was no significant difference between the proportion of the APOBEC3 B copy number in the E antigen positive HBV infection patients group and E antigen negative HBV infection patients group.Conclusions The CNV of APOBEC3 B gene is closely related to the different outcomes after HBV infection.The reduction and deletion of the APOBEC3 B gene number may be the genetic susceptibility factors for the chronicity of HBV infection and the progression of the disease.
