Part 1 A study about Expression of TLR4 receptor and its mediated inflammatory factors in placenta and peripheral blood of gestational diabetic mellitusObjective: This study examined the placenta、umbilical cordand peripheral blood Toll-like receptors 4(TLR4)and inflammatory factors expression,to explore the role of TLR4 signaling pathway in the development of gestational diabetes mellitus(GDM).And find a theoretical basis for its prevention,detection and treatment.Methods: Adult rats were selected and overnight caged.Females were randomly divided into GDM group and normal pregnancy group after confirming pregnancy.GDM group was established GDM rat model by STZ method + high fat and high sugar diet.Normal pregnant group was given the same amount of normal saline + normal feed.During pregnancy,the blood glucose of pregnant rats was monitored.Rats with blood glucose levels were cesarean section on the 19 th day of pregnancy,and rat placental、umbilical cordand tissue and peripheral blood samples were collected.The expression of TLR4 protein in rat placenta was detected by immunohistochemistry.The expression of TLR4 mRNA in peripheral blood mononuclear cells was detected by RT-PCR.The content of TNF-α and TGF-β1 in peripheral blood was detected by ELISA.Results: The expression of TLR4 in GDM group and normal pregnancy group was statistically significant(P<0.05).The expression of TLR4 in placenta and umbilical cord in GDM group was statistically significant(P<0.05);The expression of TLR4 mRNA in peripheral blood mononuclear cells of GDM group and normal pregnancy group was statistically significant(P<0.05).The difference of inflammatory factor TNF-α,TGF-β1GDM group and normal control group was statistically significant(P<0.05).Conclusion: 1.The TLR4 protein was expressed in the placenta and umbilical cord tissues of GDM group and normal pregnancy group,and the expression level of GDM group was higher than that of normal pregnancy group.The expression of umbilical cord in GDM group was higher than that of placenta,but the expression level of the normal pregnancy group was not statistically significant.2.The gene expression of TLR4 mRNA in peripheral blood mononuclear cells was higher in the GDM group than in the normal pregnancy group.3.The expression of inflammatory factors TNF-α and TGF-β in GDM group was higher than that in normal pregnancy group.Part 2 Expression of TLR4 in placental tissue and peripheral blood of pregnant women with gestational diabetesObjective: By detecting the expression of TLR4 in the placental tissue and peripheral blood of pregnant women with gestational diabetes,to verifiy that the TLR4 signaling pathway-inflammatory factors play an important role in the development of GDM,providing theoretical basis for prevention,treatment and disease judgment of GDM in clinical work.Methods:From January 2017 to January 2018,pregnant women with gestational diabetes were enrolled in the obstetrics department of the Affiliated Hospital of North Sichuan Medical College,and 12 of them were selected as experimental groups.pregnant women without pregnancy complications were collected from the same period,and 10 patients were selected as the control group.Peripheral blood was collected from pregnant women at admission.Both groups were terminated by cesarean section before labor,and placental tissue was collected during operation.The expression of TLR4 in placenta was detected by immunohistochemistry.The expression of TLR4 in peripheral blood was detected by Western blot.Results: The expression of TLR4 was detected in both the experimental group and the control group.The staining of TLR4 in the experimental group was significantly deeper and wider.There was no significant difference in the expression of TLR4 protein of peripheral blood between the experimental group and the control group,P>0.05Conclusion: TLR4 can be expressed in both placenta of GDM pregnant women and normal pregnant women,but the expression of GDM placenta is more.The clinical results are consistent with animal experiments.
