Clinical Study Of Tripterygium Wilfordii Polyglycoside Tablets In The Treatment Of Chronic Nephritis Proteinuria

Objective: The purpose of this study was to evaluate the efficacy and safety of tripterygium wilfordii polyglycoside tablets in reducing proteinuria in patients with chronic nephritis.Methods: Select 80 cases of patients with chronic glomerulonephritis proteinuria(24-hour urine protein quantitation is less than 1.0g/24h),of traditional Chinese medicine,tripterygium wilfordii(tw))60 mg/d,points three oral,cooperate with hydrochloric acid that split 10 mg/d,oral day 1 time,in three months for a period of treatment,observing a period of treatment,were observed before and after treatment for the main symptoms and signs,urine protein quantitative changes.Results:(1)a total of 80 cases were enrolled,and 2 cases fell off due to the course of treatment less than 3 months,and a total of 78 patients with chronic nephritis completed the whole course of treatment.(2)80 cases of patients with the syndrome differentiation of traditional Chinese medicine is divided into 14 of the spleen and kidney deficiency type,spleen and kidney empty carry water wet in 2 cases,spleen and kidney empty carry syndrome 18 cases,14 cases of spleen and kidney Yang deficiency type,spleen and kidney Yang deficiency type,1.25% for lung-kidney qi deficiency type,1.25% for liver-kidney Yin deficiency type.Spleen-kidney qi deficiency with damp-heat type accounts for the highest percentage and is the mostcommon clinically.(3)in the efficacy evaluation of tripterygium wilfordii polyglycoside tablets in the treatment of chronic nephritis proteinuria,a total of 78 patients,14 cases of clinical control,34 cases of significant effect,22 cases of effective,8 cases of ineffective,total effective rate of 89.74%;After 24 h of quantitative treatment,the urine protein was 0.79 0.22 before treatment and reduced to 0.41 0.21 after treatment.Paired t-test showed that P=0.000 < 0.01,showing a significant difference.The total effective rate of spleen-kidney qi deficiency type was 84.62%,that of spleen-kidney Yang deficiency type was 92.86%,that of spleen-kidney qi deficiency type was 94.44%,that of spleen-kidney Yang deficiency type was 93.75%,and that of spleen-kidney Yang deficiency type was 83.33%.Tripterygium wilfordii polyglycosides tablets has a very significant clinical effect in the treatment of chronic nephritis proteinuria.The most significant clinical effect is to reduce the spleen and kidney qi deficiency carrying damp-heat proteinuria,and the total effective rate is the highest.(4)in the comparative evaluation of the curative effect of tripterygium wilfordii polyglycoside tablets in improving the TCM syndromes of patients with chronic nephritis,the total number of patients was 78,including 19 cases of clinical recovery,26 cases of obvious effect,22 cases of effective effect and 11 cases of ineffective effect,with a total effective rate of 85.90%.The TCM syndrome integral was 18.49 2.72 before treatment and reduced to 6.36 5.43 after treatment.The paired t-test showed that P=0.000 < 0.01,showing a significant difference.The total effective rate of spleen-kidney qi deficiency type was84.62%,that of spleen-kidney Yang deficiency type was 85.71%,that of spleen-kidney qi deficiency type was 88.89%,that of spleen-kidney Yang deficiency type was 87.50%,and that of spleen-kidney Yang deficiency type was 75.00%.Through the treatment of tripterygium wilfordii polyglycosides tablets,the clinical effect of improving the TCM syndromes of patients with chronic nephritis is very significant,the clinical effect of spleen and kidney qi deficiency with damp-heat type is the most significant,the total effective rate is the highest.(5)The incidence of adverse reactions of tripterygium glycosides in the treatment ofchronic nephritis proteinuria is about 25.64%,and the incidence of bloating and stomach pain is about 12.82%,which occurs within 1 week after drug treatment.Or disappeared after reduction;the incidence of abnormal liver function(elevation of transaminase)is about3.84%,which occurs at 1-2 weeks after treatment,and returns to normal after oral administration of Huo Ning tablets;the incidence of menstrual disorders(menstrual delay,menopause)It is about 6.41%,which occurs when the treatment lasts for 1 month,and then returns to normal after the reduction;the adverse reactions can be eliminated after symptomatic treatment and reduction treatment,and the remission rate of adverse reactions is as high as 100.00%.Conclusion: Chronic nephritis patients with spleen and kidney qi deficiency dampness heat type is the most common.Tripterygium glycosides tablets have significant clinical effects in treating chronic nephritis proteinuria and improving TCM syndromes.The total effective rate of treating spleen and kidney qi deficiency and dampness heat is the highest;the incidence of adverse reactions in the treatment of tripterygium wilfordii tablets is low,mainly in the stomach.Swelling stomach pain,elevated transaminase and delayed menstruation or menopause,the cure rate of adverse reactions is as high as 100.00%,usually symptomatic treatment,complete disappearance after drug reduction.Tripterygium glycosides can effectively reduce the proteinuria of chronic nephritis,improve the clinical symptoms of patients,and delay the progression of renal failure.Clinical treatment is effective,safe and economical,and it is worthy of further research and promotion by clinicians.
