Therapeutic Effect Of Nalmefene And Naloxone On Patients With AECOPD Complicated With Mild To Moderate Pulmonary Encephalopathy

Purpose:To investigate whether nalmefene hydrochloride and naloxone hydrochlorideare effective in patients with acute exacerbation of chronic obstructive pulmonary disease and mild to moderate pulmonary encephalopathy.Method:Seventy-seven patients with acute exacerbation of chronic obstructive pulmonary disease with pulmonary encephalopathy admitted to the Second Affiliated Hospital of Nanchang University from January 2013 to January 2018 were enrolled.23 patients who was treated with routine treatment(including non-invasive ventilator assisted ventilation,anti-inflammation,antispasmodic,antiasthmatic,maintaining acid and alkal、water and electrolyte balance,etc.)+nalmefene(0.1-0.2mg twice daily),listed as group A(nalmefene group),26 patients who choose routine treatment+naloxone(Two to three milligrams two to three times a day)were setted as group B(naloxone group),and 28 patients who received conventional treatment were classified as group C(control group).Blood gas analysis values(including oxygen partial pressure,carbon dioxide partial pressure,PH),heart rate(HR),and respiratory rate(R)were collected before and on the third day of treatment,and the daily consciousness and cognitive function of the patients were collected.Kelly-Matthay scores,and the time(T)taken to reduce the patient score to 1 point was recorded,and the APACHE II scores of each patient before treatment and on the third day of treatment were recorded.Result:There were no significant differences in age and gender between the three groups(P>0.05).There were no significant differences in PH value,partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),heart rate(HR),respiratory rate(R),Kelly-Matthay score,and APACHE II score between the three groups(P>0.05).Compared with before treatment,the PaO2 and PH values of the three groups were significantly increased after treatment for 3 days(P<0.05),and the PaCO2,HR,R,and APACHEII scores were significantly lower(P<0.05).After 3 days of treatment,they were associated with the C group.Compared with group A and group B,PaO2 and PH value were significantly higher(P<0.05),PaCO2,HR,R,T,APACHEII scores were significantly lower(P<0.05).Compared with group B,group A PaO2,PH The values were significantly higher(P<0.05),and the PaCO2,HR,R,T,and APACHEII scores were significantly lower(P<0.05).Conclusion:Nalmefene hydrochloride and naloxone hydrochloride have obvious curative effect on patients with acute exacerbation of chronic obstructive pulmonary disease and mild to moderate pulmonary encephalopathy,and nalmefene hydrochloride is superior to naloxone hydrochloride.
