Objectivesealt management of gestational diabetes mellitus(GDM)patients wit tree micro-management models,in order to improve patients’ability to control blood sugar,improve pregnancy outcomes,and promote maternal and cild ealt.MetodsTe GDM patients wo were screened in te obstetrics clinic of tertiary general ospital in Zengzou City from September 1,2017 to November 1,2017 were selected as te control group.GDM patients screened in te obstetric clinic from Marc 30,2018to June 30,2018 were selected as te intervention group.Te intervention group entered te control group after full delivery and discarge.Te control group was given routine ealt education,and te intervention group was given ealt guidance of tree micro-management scemes on te basis of te control group.Te self-management questionnaire for gestational diabetes mellitus,te exercise self-efficacy scale for pregnant women and te pregnancy-related anxiety scale were used to conduct a questionnaire survey to evaluate te blood sugar canges of patients every week and record te pregnancy outcomes.Data were double-input by Excel 2016 and analyzed by SPSS 17.0 statistical software.Descriptive statistical analysis,t test,χ~2 test,rank sum test and repeated measurement variance analysis are te main statistical metods used.Results1.A total of 92 pregnant women wit gestational diabetes were enrolled in tis study.Te effective data were 86 cases,including 44 cases in te intervention group and 42cases in te control group.Tere were no significant differences in age,education level,BMI,montly income per capita,payment metod of medical expenses,etnicity,occupation,primipara,family istory of diabetes,OGTT test results(P>0.05).Te OGTT test results sowed tat te mean fasting blood glucose was 5.03±0.83mmol/L,and te mean blood glucose levels after oral administration of glucose for 1 our and 2ours were 10.30±2.44mmol/L and 8.90±1.53mmol/L,respectively.2.In tis study,independent sample t test was used to compare te blood sugar values of te two groups before and after tree meals 10 weeks after intervention.Te blood sugar values before tree meals and two ours after tree meals 4 weeks after intervention,10 weeks after intervention ad statistical significance(P<0.05).Te canges of blood sugar wit time in te two groups were analyzed by repeated measurement variance analysis.Te results sowed tat tere were significant differences in blood glucose before meals and 2 ours after meals between te two groups(P>0.001),and te blood glucose levels sowed a downward trend during te intervention.Ci-square test was used to compare te number of people wo met te standard of blood sugar before meals and two ours after meals in te first week,te tent week of intervention.Te results sowed tat te number of people wo met te standard of blood sugar control before meals and two ours after meals in te tent week of intervention was iger tan tat in te control group,wit statistical significance(P<0.001).3.Pregnancy outcomes of two groups of pregnant women were compared.Premature rupture of membranes,pregnancy-induced ypertension,polyydramnios,macrosomia and neonatal ypoglycemia were statistically analyzed by ci-square test.Te results sowed tat tere were significant differences between te two groups(P<0.05).Tere were no significant differences in postpartum emorrage,postpartum infection,intrauterine distress and mode of delivery(P>0.05).4.Comparing te self-management ability of pregnant women between te two groups,te scores of self-management ability of gestational diabetes mellitus were statistically analyzed by t-test.After intervention,te scores of eac dimension of self-management ability of pregnant women in te two groups were significantly different(P<0.05).5.Te motor self-efficacy scores of pregnant women in te two groups were tested by independent sample t test.Tere were significant differences in tree dimensions between te two groups(P<0.05).Items:I am confident to find a suitable way to exercise during pregnancy.Wen I feel tired during pregnancy,I am confident to exercise.Wen I feel depressed,I am confident to exercise.After I stop exercising for a period of time,I am confident to encourage myself to start exercising again.Tere is no significant difference(P>0.05).6.Comparing te pregnancy-related anxiety between te two groups,te scores of pregnancy-related anxiety between te two groups were statistically analyzed.Te results sowed tat tere were significant differences in te dimensions of self-concern and fear of te fetus(P<0.05).ConclusionTe tree-micro-management model based on te science-acceptance model teory can improve te self-management ability of gestational diabetes patients,reduce pregnancy-related anxiety symptoms,lower blood sugar levels,and promote maternal-cild armony.Terefore,te tree-micro management model based on te tecnology model teory can be applied to pregnant women wit gestational diabetes,and te effect is good.
