Efficacy And Safety Of Berberine Hydrochloride In Patients With Dyslipidemia Complicated With Atherosclerosis

Objective:The purpose of this study was to analyze the clinical efficacy of berberine hydrochloride in patients with dyslipidemia and arteriosclerosis by monitoring the changes of plasma lipids,non-invasive arteriosclerosis and carotid ultrasound.Meanwhile,functional results were monitored to observe the safety of the drug so as to provide a sufficient theoretical basis for its application as a multi-effect anti-atherosclerotic drug.Methods:Between June 2017 and June 2018,case collection was performed in the outpatient department of the Cardiovascular Diseases Treatment Center of the First Hospital of Jilin University.The arteriosclerosis and carotid ultrasound examination were performed in patients with abnormal lipid metabolism.Patients with carotid plaque confirmed by carotid ultrasound were enrolled in the study,and all of them were volunteered to participate in this clinical observation.All patients were given long-term use of the original drug which did not affect blood lipids and kept the low-fat diet unchanged.They were given oral berberine hydrochloride 1.0 g/d(0.5 g 2/ day).At the time of before taking the medicine,taking the medicine for 2 months and taking the medicine for 4 months,the venous blood sample was collected before oral administration of berberine and sent for examination to detect the changes of blood lipid,fasting blood glucose,serum uric acid,liver and kidney function.The results of non-invasive arteriosclerosis,carotid artery colorectal intima-media thickness,plaque Crouse score and carotid plaque area were recorded before and after 4 months of treatment.Follow-up was recorded for adverse events and endpoint events.The lipid-lowering effect of berberine was evaluated by observing changes in blood lipid levels during the medication.The changes in non-invasive arteriosclerosismeasurement and carotid ultrasound were observed to determine the therapeutic effect of berberine on arteriosclerosis.For patients with diabetes,the changes in fasting blood glucose during the treatment were analyzed without changing the hypoglycemic regimen and the diet was strictly controlled.The results of serum uric acid during the treatment were compared to analyze the effect of berberine.The changes in liver and kidney function and the occurrence of adverse reactions were recorded to evaluate the safety of the drug.All data were processed by the SPSS25.0 statistical software package.Friedman rank sum test was used to analyze multiple sets of related samples with non-normal distribution.Comparing the measurement data of the paired design,the paired t-test was used for the indicator whose difference was in accordance with the normal distribution,and the indicator whose difference did not conform to the normal distribution was applied by Wilcoxon rank sum test analysis.P<0.05 indicates that the difference was statistically significant.Results:1.There were differences in serum total cholesterol(TC)and triglyceride(TG)levels between the three stages before and after treatment(P<0.01).After 2 months and4 months of treatment,the TC and TG levels of the patients decreased,and the difference was statistically significant(P<0.05).There were no significant differences in serum high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)between the three stages before and after treatment(P>0.05).The effective rate of drug was 70%.2.After 4 months of treatment,there was no significant change in the brachial-ankle pulse wave velocity(baPWV)and the ankle brachial index(ABI)(P>0.05).3.After treatment,the intima-media thickness and the Crouse score of carotid plaque decreased,the difference was statistically significant(P<0.05),but the carotid plaque area did not change significantly(P>0.05).4.In patients with diabetes,the fasting blood glucose(FBG)decreased during the treatment but the difference was not statistically significant(P>0.05).There were differences in fasting serum uric acid index between the three stages before and aftertreatment(P<0.01).The serum uric acid(UA)was gradually decreased after 2 months and 4 months of treatment(P<0.05).5.Safety assessment: serum aspartate aminotransferase(AST)and alanine aminotransferase(ALT)levels decreased during the treatment(P<0.01).Blood urea nitrogen and serum creatinine decreased slightly,but the difference was not statistically significant(P>0.05).No serious adverse reactions and side effects were found during or after treatment.Conclusions:1.Berberine hydrochloride has a good lipid-lowering effect,and has the function of stabilizing carotid plaque.There was no significant effect on blood glucose in diabetic patients,but it can reduce serum uric acid levels.2.Berberine hydrochloride has no adverse effects on liver and kidney function,and can even reduce transaminase related indicators.It has good safety and no severe adverse reaction.
