| Objective:To investigate the significance of bone marrow immunopathology in the diagnosis and antidiastole of lymphoma,and to compare the clinical value of different detection methods in the diagnosis and staging of lymphoma.Methods:Clinical data of 343 patients with newly diagnosed lymphoma who were admitted to Department of Hematology,the First Hospital of Jilin University from August,2015 to August,2018 were retrospectively analyzed,including clinical features,pathological types,the results of different detection methods and correction of clinical stages.SPSS 22.0software was used for statistical analysis andχ~2 test was used to compare the positive rate of diagnosing lymphoma bone marrow infiltration with different detection methods.Results:1.The percentage of bone marrow infiltration in 343 cases of lymphoma was 49.3%(169/343).The bone marrow infiltration rates of NHL(non-Hodgkin’s lymphoma)and HL(Hodgkin’s lymphoma)were52.0%(169/325)and 0.0%(0/18).The rate of bone marrow infiltration was 52.7%(144/273)and 48.1%(25/52)in B type and T type NHL separately.The incidence of bone marrow infiltration in lymphoma was high,especially in mature B-cell type of non-Hodgkin’s lymphoma.The rate of bone marrow infiltration was 84.7%(122/144).2.In the 343 patients,88 cases(25.7%)of the patients were diagnosed with lymphoma by bone marrow detection on the condition that no lymph node pathology was available.87 cases(25.4%)were identified by bone marrow immunopathology and 83 cases(24.2%)by bone marrow flow cytometry in definitely diagnosing pathological classification.The pathological types of lymphoma can be identified by using bone marrow immunopathology and bone marrow flow cytometry to complement each other.3.The positive rate of bone marrow infiltration detected by bone marrow smear,bone marrow biopsy,bone marrow flow cytometry and bone marrow immunopathology in 169 patients were 78.7%(133/169),82.8%(140/169),91.1%(154/169),97.6%(165/169)separately.The positive rate of bone infiltration detected by PET/CT in 68 patients was39.7%(27/68).Bone marrow immunopathology was more advantageous and was statistically different from the rest of detection methods(χ~2=27.250、19.343、5.577、105.778;P<0.05).Bone marrow flow cytometry was more advantageous and was statistically different from bone marrow smear,bone marrow biopsy and PET/CT(χ~2=10.184、5.121、71.042;P<0.05).Bone marrow immunopathology was the most sensitive method for detecting lymphoma bone marrow infiltration.4.Among the 343 patients,169 of them were identified with bone marrow infiltration by bone marrow detection and the staging of 61patients(36.1%)were amended.Among them,43 cases(25.4%)were amended by bone marrow smear,47 cases(27.8%)by bone marrow biopsy,58 cases(34.3%)by bone marrow immunopathology,55 cases(32.5%)by bone marrow flow cytometry.Bone marrow examination can correct clinical staging of lymphoma,and the correct rate of bone marrow immunopathology in clinical staging was the highest.Conclusions:1.The incidence of bone marrow infiltration in lymphomas was high,mainly non-Hodgkin’s lymphoma.2.Bone marrow immunopathology showed the highest sensitivity to lymphoma bone marrow infiltration and the highest correction rate for clinical staging.3.Some lymphoma can be detected by bone marrow examination to identify specific pathological classification. |
