Background Gastric cancer is one of the common malignant tumors of the digestive tract,which brings heavy burden to the society and families and poses a great threat to individual health.CAG with intestinal metaplasia or dysplasia is considered as a precancerous lesion of gastric cancer.H.pylori infection and methylation of Runx3 gene are closely related to CAG and gastric cancer.Objective To observe the relationship between Runx3 expression of gastric mucosa and H.pylori infection in CAG.To investigate the effect of h.pylori infection on the expression of Runx3 methylated mRNA and protein in gastric mucosa of CAG patients.Methods Eighty patients with chronic atrophic gastritis were selected and divided into observation group of 53 CAG with H.pylori infection and control group of 27 CAG without H.pylori infection.The mRNA and protein expression of two groups were compared.All patients of observation group were receiving H.pylori eradication therapy.The expression change of mRNA and protein before and after treatment were compared.Results Among the 80 patients with moderate to severe CAG included in this study,36 patients had positive methylation of Runx3 gene in gastric mucosa,with a total positive rate of 45.0%.The positive methylation rates of Runx3 in the control group and the observation group were 29.6% and 52.8%,respectively.The difference between the two groups was statistically significant(p<0.05).The mRNA expression of Runx3 gene in gastric mucosa of patients with CAG in the observation group was lower than that of the control group,but the difference was not statistically significant(p>0.05).Subgroup analysis showed that mRNA expression of Runx3 gene in gastric mucosa of patients with severe CAG was lower than that of the control group(p<0.05).Protein expression of Runx3 gene in gastric mucosa of patients with CAG in the observation group was lower than that of the control group(p<0.05).Subgroup analysis showed that protein expression of Runx3 gene in gastric mucosa of patients with moderate or severe CAG was lower than that of the control group(p<0.05).Forty-four patients became negative after eradication of H.pylori in 53 patients of observation group,and the eradication rate was 83.0%.Among the 44 patients with H.pylori,6 were negative after eradication in the positive of Runx3 methylation.After the eradication of H.pylori,mRNA and protein expressions of Runx3 gene in gastric mucosa of patients with CAG were higher than the baseline,and the differences were statistically significant(p<0.05).Subgroup analysis showed that mRNA and protein expressions of Runx3 gene in gastric mucosa of patients with moderate or severe CAG were higher than the baseline,and the differences were statistically significant(p<0.05).Conclusion The positive rate of Runx3 methylation in patients with CAG was high,and the H.pylori infection may lead to increased methylation in patients with moderate to severe CAG.H.pylori infection can reduce the mRNA and protein expression of Runx3 in patients with CAG,and the expression were up-regulated after H.pylori eradication.
