| Objective To understand the current situation of pre-operative blood glucose control patients with type Ⅱ diabetes,and to analyze the related factors of substandard blood glucose control from the perspective of patients’ own KAP,then provide reference and basis for exploring effective KAP related nurse and preventive measures for pre-operative blood glucose control patients with type Ⅱ diabetes.Methods From March 2016 to March 2018,we selected 200 patients with type Ⅱ diabetes who were admitted to the department of surgery of the Three-A general hospital in Jinzhou city before surgery,and conducted a descriptive study and cross-sectional survey to study the current situation of patients’ blood glucose control and analyze the KAP factors affecting blood glucose compliance.After signing the informed consent form,they filled in the general information questionnaire,the knowledge scale,the attitude scale and the self-care scale for patients with diabetes were filled out.After the investigation questionaires were collected,using excel double input data to build a database,using SPSS22.0 express the measurement data with mean±standard deviation.The counting data were described with frequency and percentage at the same time descriptive analysis and correlation analysis.A total of 200 questionnaires were distributed throughout the whole process,and 143 valid questionnaires were recovered,with an effective recovery rate of 71.50%.Results 1.Patients with type Ⅱ diabetes mellitus,the fasting blood glucose was 10.28±2.99mmol/L,at 2 hours after meal.blood sugar was12.65±3.29 mmol/L and the glycosylated hemoglobin was 8.53±1.53%.The rate of fasting blood sugar reaching the standard was 11.90%,the at 2 hours after meal.blood sugar reaching the standard was 2.80%,The rate of reaching the standard at the same time was only 1.40%.,at the same time,the rate of reaching the standard was only 1.40%,overall,preoperative blood sugar management in patients with type 2 diabetes mellitus is not ideal.2.The dimensional Score of Knowledge Scale of diabetic patients(Adults)The total score of knowledge scale of diabetic patients(adults)was 68.33 ±5.47,among them,in the dimension of illness in the lowest,the score was 4.01±1.82,the dimension of foot care in the highest,the score was14.23±5.47.The average score of each dimension from high to low was foot care,diet/food,hypoglycemia,reducing the risk of complications,diabetes treatment,the influence of smoking/drinking,physical exercise and illness.Overall,the knowledge level of diabetes mellitus patients before operation needs to be improved.3.The dimension score of the diabetes attitude scale The total score of the diabetes attitude scale was 115.53±13.29,among them,in the dimension of the average score of the necessity professional training in the lowest,the score was 20.15±3.11,the dimension of Patient’ autonomy in the highest,the score was 28.19±4.80.The average score of each dimension from high to low was patient autonomy,the severity of type Ⅱ diabetes,the value of strictly controlling blood sugar,the psychological impact of diabetes mellitus and the necessity of professional training,overall,the attitude of type Ⅱ diabetes patients needs to be further improved.4.The score of each dimension of diabetes self-care behavior scale The total score of self-care behavior scale for diabetes mellitus was 65.20 ±21.06,among them,in the dimension of the average score of the smoking in the lowest,the score was 0.23±0.42,the foot nursing behavior in the highest,the score was 18.92±8.75.The average score of each dimension from high to low was foot care behavior,dietary behavior,medication behavior,blood sugar monitoring behavior,exercise behavior,smoking,overall,preoperative diabetic self-management behavior of patients with type Ⅱ diabetes needs to be improved.5.Pearson’s correlation analysis and Kendall’s supplementary correlation analysis showed that there was a positive correlation between personal knowledge and personal attitude among knowledge,trust and practice(r>0.5,P<0.01).There is a positive correlation between personal knowledge and personal behavior(r>0.5,P<0.01);There is also a positive correlation between personal attitude and personal behavior(r>0.5,P<0.01).Conclusions The current situation of blood glucose control in patients with type Ⅱ diabetes prior to surgery was not optimistic.The improvement of blood glucose control in diabetic patients can be promoted from three aspects: the level of knowledge related to diabetes,attitude towards diabetes,and daily self-care behavior of diabetes. |
