ObjectiveTo assess and investigate the efficacy and safety of recombinant human growth hormone(rhGH)in the treatment for short stature children and analysis of comprehensive management in the treatment. It provides a reliable basis for the clinical application of rhGH in the treatment of GHD and ISS children.MethodsA total of 63 children with GHD and ISS short stature were enrolled in the study from 2016 to 2018 in pediatrics department of our hospital. They were divided into GHD group(0.12~0.15IU/kg daily)and ISS group(0.15IU/kg daily).Every night before going to bed,rhGH was administered by subcutaneous injection. It also has comprehensive management(diet and nutrition guidance, reasonable exercise,high-quality sleep, physiological and psychological effects)and oral vitamin D3 400 IU everyday. The changes and correlations of the clinical indicators of serum levels of insulin-like growth factor-1(IGF-1),height velocity(HV),height standard deviation score(Ht SDS),bone age(BA),chronological age(CA),height age(HA)and fasting blood glucose(FBG)after rhGH treatment were analyzed between the two groups.Results1.Before treatment,HV differences between the two groups were statistically significant(P<0.05),while the remaining differences were not statistically significant(P>0.05).Ht SDS and HV both increased after the treatment with two groups,Ht SDS and HV in the GHD group were significantly higher than in the ISS group 6,9,and 12 months after treatment(P<0.05).2.Both group increassed serum levels of IGF-1,The serum IGF-1 value of GHD group was higher than that of ISS group, and the difference between the two groups at 6,9 and 12 months after treatment was statistically significant(P<0.05).Serum levels of IGF-1 were positively correlated with Ht SDS in both groups.3.There was no statistically significant difference in the progression of 1 years of bone age between the two groups(P >0.05).4.One case had a headache. One child each in the two groups experienced transient slight increase in fasting blood groups(6.1mmol/L).Conclusions1.rhGH has a good effect on the growth promotion of GHD and ISS short stature children, the growth rate of GHD is better than ISS.2.In the treatment of rhGH,IGF-1,bone age and FBG should be monitored regularly.3.rhGH treatment does not lead to bone age maturation and premature puberty.4.rhGH has less adverse reactions in the treatment, and good comprehensive management can make the drug effect play to the utmost, and meanwhile developing healthy habits of children is conducive to their physical and mental health.
