| Objective:Small cell lung cancer(SCLC)is a rapidly growing and highly invasive malignant tumor,which is crucial for early diagnosis and treatment to improve the prognosis of patients.This study will investigate the significance of combining serum specific enolase(NSE)and Lactate dehydrogenase(LDH)on the diagnosis and efficacy evaluation of small cell lung cancer.METHODS:This study retrospectively collected during January 2016 to December 2018 in the first affiliated hospital of nanchang university by pathological diagnosis of small cell lung cancer patients with clinical data,including gender,age,smoking history,clinical stage,serum NSE,serum LDH,and record a gleam of platinum chemotherapy after 2 cycles of serum NSE,LDH and curative effect evaluation,and collect in the same period in the hospital of non-small cell lung cancer(Non-small cell lung cancer,NSCLC)patients and lung Benign lesions(Benign lung diseases,BLD),including gender,age,smoking history,clinical stage,serum NSE,serum LDH.SPSS 25.0 was used for statistical analysis of all data,and P<0.05 was considered statistically significant.Result:1.The median NSE level before chemotherapy in SCLC group,NSCLC group and BLD group was 40.17ng/ml,20.97ng/ml and 15.52ng/ml,respectively.The serum NSE levels in the three groups were statistically significant(P<0.001).After comparing them in pairs,the difference of serum NSE level between SCLC group and NSCLC group,SCLC group and BLD group was statistically significant(P<0.001,P<0.001,P=0.046).The median LDH levels in the SCLC group,NSCLC group and BLD group before chemotherapy were 287.50 U/L,240.00 U/L,and 212.00U/L,respectively.The serum LDH levels in the three groups were statistically significant(P< 0.001),after comparing them in pairs,serum LDH levels werestatistically significant in SCLC group and NSCLC group,SCLC group and BLD group,NSCLC group and BLD group(P=0.040,P<0.001,P=0.046).2.The sensitivity and specificity of serum NSE diagnosis of SCLC before chemotherapy were 83.3% and 73.0%,respectively.The sensitivity and specificity of serum LDH in the diagnosis of SCLC before chemotherapy were 70.0% and 69.0%,respectively.The sensitivity and specificity of serum NSE and LDH in the diagnosis of SCLC were 86.7% and 72.0%,respectively.3.Serum NSE levels before chemotherapy of patients with SCLC gender,age,smoking history,clinical stage had no significant correlation(P> 0.05),serum LDH level before chemotherapy of patients with SCLC gender,age,smoking history has no significant correlation(P> 0.05),is associated with significant clinical staging(P=0.012),it can be considered that serum LDH level in ED stage and LD stage is significantly different.4.Serum NSE and LDH levels in the SCLC group were statistically significant before and after two cycles of first-line platinum chemotherapy(P<0.001,P<0.001),so it can be concluded that there are significant differences in serum LDH levels between extensive disease(ED)and limited disease(LD).5.The difference in serum NSE and LDH levels before and after chemotherapy was statistically significant in the CR+PR group(P<0.001,P=0.001),but not in the SD+PD group(P=0.273,P=0.068).Conclusion:Combined serum NSE and LDH can assist the diagnosis of SCLC patients.Serum LDH level can be used for clinical staging of SCLC patients.The difference in serum LDH level before and after chemotherapy can be used to evaluate the efficacy of first-line platinum chemotherapy in SCLC patients. |
