| Objective: To explore the influencing factors and correlations of post-operative pregnancy in patients with endometriosis and infertility,and to compare the influence of various factors on post-operative natural pregnancy rate and the situation of post-operative natural pregnancy in patients with endometriosis and infertility,so as to provide basis for future research to improve the natural pregnancy rate in patients with endometriosis and infertility.Methods: The clinical data of female patients with infertility due to endometriosis from January 2013 to January 2016 were collected.After diagnosis by laparoscopy,the clinical data and follow-up data of 24 months after operation were summarized and summarized retrospectively.Based on the fertility of patients within 24 months after operation,the influence of endometrium was determined.Factors of spontaneous pregnancy in patients with ectopic diseases.There were 116 patients in this group at the beginning of the patients who completed the follow-up,38 were pregnant naturally,and the overall pregnancy rate was 35.84%.The natural pregnancy rate was the highest 0-6 months after the operation,which was the best time for pregnancy.Later,the natural pregnancy rate decreased with the extension of time.Age,duration of infertility,r-AFS stage and GnRH-alpha treatment after operation were the determinants of COX univariate nalysis.These factors were independent determinants of natural pregnancy outcome in patients with endometriosis.Multivariate logistic regression confirmed that age,type of infertility,post-operative GnRH-alpha treatment and modified endometriosis staging(r-AFS)were risk factors for the natural pregnancy rate of patients with endometriosis.There was a strong statistical significance between the factors.The natural pregnancy rate of endometriosis fertility index score 0-4 was 28.57%,the natural pregnancy rate of 5-7 was 53.84%,and the natural pregnancy rate of 8-10 was 77.78%.Tau-b of natural pregnancy rate and endometriosis fertility index score was 0.893,P<0.001,so the natural pregnancy rate after endometriosis operation was positively correlated with endometriosis fertility index score.In the modified endometriosis staging method(r-AFS),the natural pregnancy rate was 50.00% in stage I and 36.17% in stage II,while 35.00% in stage III and 13.33% in stage IV.The natural pregnancy rate and Tau-b=-0.285,P<0.001 in the modified endometriosis staging method(r-AFS).It can be seen that the natural pregnancy rate of endometriosis patients after operation and that of endometriosis patients after operation are higher than that of the modified endometriosis staging method(r-AFS).R-AFS staging was negatively correlated.In GnRH-a drug group,there was no significant difference in age,infertility years and stages between the two groups(P > 0.05).The natural pregnancy rate was 54.90% in the medication group and 18.18% in the patients without GnRH-a medication.The influence of medication on the natural pregnancy rate was statistically significant(P < 0.001).There were 4 cases with adverse outcomes in the medication group,which was significantly lower than 6 cases in the non-medication group after operation(P < 0.05).Conclusion: Laparoscopic surgery can significantly improve the natural pregnancy rate of patients with endometriosis.The best period of pregnancy is 0-6 months after operation,which has a good natural pregnancy rate.After that,the pregnancy rate is negatively correlated with the time.Postoperative pregnancy of infertility patients with endometriosis is significantly correlated with patient’s age,infertility years,LF score and GnRH-alpha drug therapy.Surgery can improve the natural pregnancy rate of infertility patients with EMT.It is not only an effective method to treat infertility,but also a golden way to diagnose endometriosis.Postoperative pregnancy rate was positively correlated with the fertility index score of endometriosis and negatively correlated with r-AFS staging.Taking GnRH-alpha after EMT for infertility patients can improve the natural pregnancy rate and reduce the incidence of adverse pregnancy.In clinical work,according to the patient’s age,infertility years,LF score,EFI score and r-AFS stage according to the patient’s physiological and reproductive requirements,appropriate auxiliary measures should be taken for patients to improve the natural pregnancy rate and meet the patient’s fertility.Desire. |
