Effect Of Warm Acupuncture And Moxibustion Comprehensive Intervention On Quality Of Life Of Patients With Gastric Cancer Undergoing Chemotherapy

ObjectiveBased on the routine treatment and nursing measures,the comprehensive intervention measures of warming acupuncture and moxibustion were carried out,which included needling,moxibustion nursing,living at four seasons and dietary guidance based on syndrome differentiation.By observing the gastrointestinal reactions,anorexia,sleep disorders and other common concomitant diseases of patients with gastric cancer undergoing chemotherapy,this paper analyzed the effect of comprehensive intervention of warming needle moxibustion on the quality of life of patients with gastric cancer undergoing chemotherapy and provided the basis for seeking effective comprehensive nursing measures for patients with gastric cancer undergoing chemotherapy.MethodThis study was examined by the Institutional Review Board.126 patients with gastric cancer who received mFOLFOX6(oxaliplatin + calcium folinate + 5-fluorouracil)chemotherapy in 6 wards of 2 departments of Henan Cancer Hospital from October 2017 to May 2018 were selected to participate in this study.They were divided into treatment group and control group according to the different treatment methods voluntarily accepted by patients.63 patients in each group were divided into two groups,of which 61 patients in the treatment group were withdrawn from the study for reasons.In the control group,62 patients completed the study by interrupting treatment before discharge.The control group received routine mFOLFOX6 chemotherapy regimen,western medicine,stomach care,antiemetic drugs and psychological care,health education,dietary care and other routine nursing measures.On the basis of the control group,the treatment group was given two cycles of warm needling and moxibustion combined with dietary guidance according to the four seasons.According to the twelve meridians corresponding to twelve hours and meridian flow injection method,the most active time of stomach meridians(7-9 hours)was selected for treatment.Acupuncture method: Disinfect the skin in the area of acupoint selection,select appropriate type of filiform needle according to the location of the needle,hold the needle correctly,stab the door acupoint at the angle of needle insertion,stab other acupoints directly,cut fresh ginger into thin slices and open a small hole in the center to place the skin around the acupoint through the filiform needle,and insert the moxa stick about 2 cm long on the needle handle to light moxibustion,each time.The needle retention time was 30 minutes once a day,and 7 days was a treatment cycle.This study treated two cycles,a total of 14 times.Two groups of patients completed the information collection of general information questionnaire,common symptoms of tumors and TCM symptoms questionnaire,gastrointestinal reaction grade and core quality of life scale before and after treatment.Data were recorded and analyzed by SPSS19.0 software.The general demographic data of patients were described by frequency and percentage.Chi-square test was used for comparison between groups.The scores of each item and each field in the MDASI-TCM and EORTC QLQ-C30 before and after treatment were expressed by mean(±)standard deviation.Result Independent sample t test was used to describe the change of gastrointestinal reaction grade and the effective rate of curative effect evaluation between the two groups.Rank sum test was used to test the level of ?=0.05.Result1.Both groups of patients had different degrees of nausea and vomiting during chemotherapy.There was no significant difference in the number of nausea and vomiting between the control group and the treatment group(P > 0.05).However,the number of patients with nausea and vomiting in the treatment group was lower than that in the control group(P < 0.05),and the severity of vomiting was lighter than that in the control group(P < 0.05).2.There was a significant difference in the severity of some common cancer symptoms between the two groups(P < 0.05).The severity of pain,sleepiness,loss of appetite,diarrhea,constipation and restlessness in the treatment group was lighter than that in the control group.At the same time,the distress degree of symptoms on daily activities in the treatment group was lighter than that in the control group.3.The total quality of life scores of the treatment group and the control group before chemotherapy was(54.56 ±11.47)and(52.41 ±9.36),respectively.There was no significant difference(P > 0.05).The overall quality of life of the two groups was in the lower middle level.There among,the lowest score in the functional field is the role function,followed by the emotional function.The overall quality of life in the treatment group was(52.60 +9.68)and that in the control group was(44.09 ±7.89),which was higher in the treatment group than that in the control group.There was a significant difference between the two groups(P < 0.05).The improvement of physical function,role function and emotional function in the two groups was statistically significant(P < 0.05).The scores of sleep disorder,anorexia,nausea and vomiting,diarrhea and constipation in the treatment group were lower than those in the control group(P < 0.05),and the improvement of symptoms in the treatment group was better than that in the control group.Conclusion1.Warm acupuncture therapy can effectively prevent the occurrence of nausea and vomiting in patients with gastric cancer during chemotherapy and reduce the severity of vomiting.2.The treatment group had a better degree of alleviating the common accompanying diseases of gastric cancer than the control group.Meanwhile,the overall quality of life of the treatment group was higher than that of the control group,which indicated that warm acupuncture could effectively improve the common accompanying diseases of gastric cancer patients undergoing chemotherapy and improve the quality of life of patients.
