The Clinical Observation Of Jiang Zhuo Tong Luo Granuies Treat Stable Angina(Syndrome Of Intermingled Phlegm And Blood Stasis)

Objective: Through observing the clinical effect of Jiang Zhuo Tong Luo GranuIes combined with conventional western medicine in treating SAP of CHD(phlegm-blood stasis mutual junction type),this study provides effective and safe medical research evidence for the prevention and treatment of SAP of CHD(phlegm-blood stasis mutual junction type),enlarges the clinical application scope of Jiang Zhuo Tong Luo GranuIes,and at the same time it provides effective clinical thinking and practical experience for the prevention and treatment of SAP of CHD by integrated traditional Chinese and Western medicine.Methods: A total of 126 patients with stable angina pectoris(phlegm-blood stasis mutual junction)and 2 patients(One patient went abroad halfway and failed to take the drug;the other patient failed to complete the clinical study because of the accident in the halfway home.)with exfoliation were selected from the outpatient department of Cardiology of Shanxi Provincial Hospital of TCM from January 2018 to June 2018.The final effective cases are 124.The patients were divided into treatment group and control group by statistical random table method.The number of cases in each group is 62.Two groups of patients were given health education,smoking cessation,alcohol cessation and other lifestyle improvement education and secondary drug prevention and treatment of coronary heart disease.On this basis,the control group took Jiang Zhuo Tong Luo GranuIes placebo,however the treatment group took Jiang Zhuo Tong Luo GranuIes orally besides the basic drug.After 12 weeks of treatment,the effects of angina pectoris score,TCM syndrome score,maximum heart rate of treadmill exercise test,exercise test level,metabolic equivalent(METs),duration of exercise test,total decrease of ST segment in exercise(mV),Seattle scale evaluation and safety quantitative index were observed to determine the clinical efficacy and safety of Jiang Zhuo Tong Luo GranuIes.Results:(1)There was no significant difference(P > 0.05)between the two groups in terms of age,sex,course of disease and angina pectoris grade;(2)Clinical score and curative effect of angina pectoris: there was significant difference between the two groups before and after treatment(P < 0.01);The treatmentgroup and the control group were different significantly(P < 0.01);After treatment,the total effective rates of control group and treatment group were 66.74% and 7.1%respectively.The two groups were different significantly(P < 0.05).The treatment group was superior;(3)Total integral of TCM syndromes and curative effect: In total integral of TCM syndromes,the treatment group and the control group were different significantly(P <0.01);there were significant differences between the treatment group and the control group after treatment(P < 0.01);After treatment,the total effective rates of control group and treatment group were 69.35% and 88.71% respectively.The two groups were different significantly(P < 0.01);The improvement of symptoms and the effect of traditional Chinese medicine were better than those of the control group;(4)After treatment,treadmill exercise test: total time of exercise,metabolic equivalent,exercise level,maximum heart rate and total index of ST-segment depression during exercise: There were significant differences in total index of ST-segment depression between the two groups before and after treatment(P <0.01).There was no significant difference in total index of ST-segment depression between the treatment group and the control group after treatment(P>0.05);other indicators have statistical significance(P < 0.01);it can be considered that the improvement of treadmill exercise test in treatment group is better than that in control group;(5)Seattle Angina Scale: There were significant differences between the treatment group and the control group in terms of physical activity limitation,stable state of angina pectoris,treatment satisfaction and disease awareness(P < 0.01),and there were significant differences between the treatment group and the control group in terms of attack of angina pectoris(P < 0.05).It can be concluded that the Seattle Scale evaluation group is better than the control group;(6)In the course of this experiment,the general physical examination items of patients,including BP(mmHg),T(℃),R(sub/score),P(sub/score),blood and urine routine,glutamic acid,ALTand AST renal function,ECG and other safety indicators,were not abnormal before and after treatment;Conclusion:Jiang Zhuo Tong Luo GranuIes have the effect of invigorating Qi and activating blood circulation,reducing turbidity and dredging collaterals in treatingSAP of CHD(phlegm and blood stasis type).The clinical symptoms of the patients have been greatly improved,improve the reserve of cardiac blood supply capacity,enhance the exercise tolerance and intensity of patients,and improve the quality of life to a certain extent without obvious adverse reactions.Jiang Zhuo Tong Luo GranuIes can be used in the treatment of stable angina pectoris(phlegm-blood stasis type).
