ObjectiveThrough drug intervention on patients with knee osteoarthritis and osteoporosis after total knee arthroplasty,to explore the effect of Kidney-Tonifying and Blood-activating method on improving functional rehabilitation after operation,and to provide reference and basis for clinical application of Kidney-Tonifying and Blood-Activating method..MethodsIn this study,patients with knee osteoarthritis and osteoporosis who were treated by total knee arthroplasty in orthopaedics department of our hospital from January 2018 to December 2018 were randomly divided into control group(basic medication)and experimental group(basic medication+Kidney-Tonifying and Blood-Activating method).The final control group had 20 cases,the experimental group had 20 cases,a total of 40 cases.Bone mineral density(BMD)was measured before operation in both groups,and Rogaiquan(1 capsule,qd,oral)and Calcium D(1 capsule,qd,oral)were given as basic drugs the next day after operation.HSS score and bone metabolism index were measured in the experimental group on the 7th day after operation,and the Kidney-Tonifying and Blood-activating method(Gusong’ an capsule(3 TID oral)was added on the basis of basic drugs.HSS functional score and laboratory bone metabolic index of knee joint were measured at 3 months and 6 months after operation,and dual energy X-ray bone mineral density was measured at 6 months after operation.Relevant data were collated and analyzed.Statistical software was used for statistical analysis.The difference of functional rehabilitation between the two groups was compared after different treatments,and the changes of bone metabolism and bone mineral density were observed.Results1.General information:There was no significant difference in age,sex,surgical site(left knee/right knee),hospitalization days between the two groups(P>0.05);there was no significant difference in BMD before operation between the two groups;there were no significant differences in vitamin D level,PINP level,CTX level,HSS score and HSS functional rating between the two groups on the 7th day after operation(P>0.05).2.HSS score of knee joint:The total score of HSS in knee joint of the two groups at 3 months and 6 months after operation was higher than that before treatment(7 days after operation),and the difference was statistically significant(P<0.05).At 3 months and 6 months after operation,the HSS score in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in HSS functional rating(excellent,medium,good and poor)between the experimental group and the control group at 3 months and 6 months after operation(P>0.05).3.Bone metabolic markers:The levels of vitamin D and β-CTX before treatment(7 days after operation)and 3 months and 6 months after operation in the control group had no significant difference(P>0.05),while the levels of PINP after 3 months and 6 months after operation decreased compared with those before treatment(7 days after operation),and the difference was statistically significant(P<0.05).There was no significant difference in the levels of vitamin D,beta-CTX and PINP before treatment(7 days after operation)and after 3 months and 6 months(P>0.05).There was no significant difference in the levels of vitamin D and β-CTX between the experimental group and the control group at 3 months and 6 months after operation(P>0.05);there was no significant difference in the level of PINP between the two groups at 3 months after operation(P>0.05),but the level of PINP in the experimental group was significantly higher than that in the control group at 6 months after operation(P<0.05).4.Bone mineral density:There was no significant difference between preoperative BMD level and 6-month BMD level(P>0.05)in both groups,and there was no significant difference between the two groups in 6-month BMD level(P>0.05).Conclusion:For the treatment of patients with osteoporosis after total knee arthroplasty,Kidney-Tonifying and blood-activating therapy combined with basic medication of Western medicine can inhibit bone absorption,promote bone formation,slow bone loss after operation,improve functional rehabilitation exercise,improve the quality of life of patients,the effect is remarkable,and it is worth popularizing and applying.
