Research On The Demand And Contents Of The Family-doctor-style Pension Integration Of Medical Care And Health Services

ObjectiveWe aim to form the content of the family doctor service through Delphi method,for the government sector to develop the family doctor service project to provide a reference.To investigate the demand of the family doctor service in the elderly,to determine the content of the family doctor service and laying the foundation for standardizing the development of the family doctor service.MethodLiterature research method:through consulting relevant literature and policy documents,preliminary to draft the content indicators.Delphi method:selecting experts from related fields to carry out two rounds of expert consultation,establish content indicators.Questionnaire survey:The elderly in Jiangan District,Qiaokou District and Dongxihu District of Wuhan City were sampled by sampling method with convenience sampling,an investigation on the demand of family doctor service in Wuhan.Epidata 3.1 was used to input data,and SPSS20.0 was used to conduct statistical analysis,such as descriptive statistics,chi-square test,and the like.ResultsThe positive coefficients of two rounds of consultation were 100% and 94.44%,respectively,and the authority coefficients of experts were 0.836 and 0.906.After two rounds of consultation,the coordination of indicators were 0.109,0.113 and 0.305,respectively(P<0.01).Through Delphi method,the contents of the family doctor service consisted of 6 first-level indicators,38 second-level indicators.The results of the survey showed that the Cronbach’s α was 0.968 and S-CVI/Ave was 0.941.According to the survey,88.3% of the elderly have different levels of the family doctor service needs,of which 40.67% of the elderly demand reached 31 and above;demand projects focus on basic medical care(make an appointment to see a doctor,dual referral,medical services,health consultation),basic public health services(establish health record,health assessment,chronic health screening,medical examination),health management(develop a health management plan,personal health information query,TCM healthcare).Conclusion(1)The aged people indeed demand the family doctor services which could meet their health needs.(2)Suitable for the Wuhan community to carry out the family doctor service project content,are made up of six kinds of services including 38 items,the six kinds of services mainly including basic medical care,basic public health services,health management,family ward,rehabilitation health services and health guidance.The pre-survey shows that it has good reliability.And the most important services which should be put into practice are basic medical care,basic public health services and health management so as to meet the basic need for chronic disease management and health pension of most elderly persons.Meanwhile,certain kinds of therapeutic nursing technique services,which are necessary and feasible,should be conducted to appease old persons’ individual care needs under diverse health conditions.(3)Through a questionnaire survey of 300 samples,it is concluded that the demand concentration of the family doctor services for the elderly with different ability levels provides a reference direction for the development of the family doctor service.
