Background:Fatigue symptom has the highest incidence of multiple physical and psychological symptoms in hemodialysis patients, and are most plagued, adversely affecting social interactions in hemodialysis patients and degrading quality of life.Resistance exercise,as a form of strengthening muscle strength,is more moderate in its exercise, and its health benefits have been widely recommended and applied in some healthy and chronic diseases. The effectiveness of the resistance exercise during hemodialysis on the intervention of fatigue symptom remains to be further studied and confirmed. Objective:To investigate the effect of resistance exercise during dialysis on fatigue symptom in maintenance hemodialysis patients during 12-week dialysis exercise and 12-week follow-up.Methods:This study was an experimental study with convenient sampling. From June 2018 to April 2019,patients who underwent maintenance hemodialysis treatment at a third-grade hospital in Beijing, and met the inclusion criteria, was selected. The group A and group B were randomly assigned to the intervention group and the control group according to two different dialysis schedules of Monday-Wednesday-Friday dialysis time(group A)or Tuesday-Thursday-Saturday dialysis time(group B).The patients in the control group received routine hemodialysis from the blood purification center. On the basis of routine hemodialysis, the patients in the intervention group performed moderate and low-intensity resistance exercise with peanut fitness balls during dialysis in 12-week period.Exercise intervention:The patient took the peanut fitness ball as resistance exercise tool and did the moderate and low-intensity resistance exercise with lower limbs for 30 minutes of between 0.5 hours and 2 hours after the start of hemodialysis treatment.During the 12-week intervention and 12 weeks after the intervention, the patient’s fatigue, muscle strength, physical activity, depression and serum albumin data were collected every 4 weeks, and the patient C-reactive protein data was collected every 12 weeks. In the second week after the intervention, the intervention exercise evaluation questionnaire was used to evaluate the intervention process. Results:After the intervention,a total of 42 subjects completed data collection. The results showed:(1)Fatigue:After 12 weeks of intervention, the FACIT-F score of the intervention group was significantly higher than that before the intervention(P<0.002),and significantly higher than the control group(P<0.05).(2)Muscle strength:After 12 weeks of intervention, the STS 10 time of the intervention group was significantly lower than that before the intervention(P<0.002),and significantly lower than the control group(P<0.05).(3)Physical activity level:After 12 weeks of intervention, the daily average number of steps of patients in the intervention group was significantly higher than that before the intervention(P<0.002),and significantly higher than the control group(P<0.05).(4)Depression:After 12 weeks of intervention, the SDS score of the intervention group was significantly lower than that before the intervention(P<0.002),and significantly lower than the control group(P<0.05).(5)Biochemical indicators:After 12 weeks of intervention, the serum albumin content and the normal condition of C-reactive protein content in the intervention group and the control group were not significantly different from those before the intervention both in groups and between groups.(6)95.2%of the subjects indicated that they were very satisfied or satisfied with the content, method and time of the intervention. Conclusion:Moderate and low-intensity resistance exercise during dialysis can improve the fatigue symptom in maintenance hemodialysis patients, enhance the muscle strength of maintenance hemodialysis patients, improve the physical activity level of maintenance hemodialysis patients and improve the depressive symptom in maintenance hemodialysis patients. This study provides a practical basis and reference for clinical staff to better maintain and promote the physical and mental health of maintenance hemodialysis patients in the future.
