| Objective:1.To understand the anxiety and depression of patients with complex anal fistula after operation and its influencing factors,and to provide theoretical basis for psychological intervention of patients with complex anal fistula after operation.2.To explore the effects of focused solution model on anxiety,depression,pain and quality of life in patients with complex anal fistula after operation,so as to provide a psychological intervention method with clinical value and practical application for improving patients’negative emotions,alleviating postoperative pain,promoting post-operative rehabilitation and improving quality of life.Methods:1.Investigation and research:From June 2017 to June 2018,216 patients with complex anal fistula who were hospitalized in Anorectal Department of Shanxi Third Class A General and Specialized Hospital were conveniently selected as the research objects.The general data questionnaire,self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were used as the research tools to investigate the anxiety,depression and their influencing factors.2.Intervention study:From July 2018 to December 2018,70 patients with complex anal fistula after operation were collected in Anorectal Department of a general hospital of Shanxi Province.They were randomly divided into experimental group and control group,35 cases in each group.Both groups received routine treatment and basic nursing of anorectal department.The control group received routine psychological nursing,while the experimental group adopted"focused solution model"psychological intervention on the basis of the control group.Anxiety score,depression score,post-operative pain score and quality of life score at 1 month follow-up were compared between the two groups before and after intervention.3.Statistical analysis:SPSS22.0 software was used for statistical processing.Investigation stage:Statistical description of the general data of the respondents was made,and the influencing factors were analyzed by t-test,one-way ANOVA and multiple linear stepwise regression analysis.Intervention stage:In the baseline data of two groups of patients,descriptive statistical analysis was made on the general data.Measurement data were compared before and after intervention with two independent samples t test and paired t test.Counting data were compared before and after intervention with?~2 test.The difference was statistically significant with P<0.05.Results:1.Among 216 patients with complex anal fistula,134 had anxiety,accounting for 62.04%of the study population,and the anxiety score was 50.99±10.17;95 had depression,accounting for 43.98%of the study population,and the depression score was 49.92±11.94.The main factors affecting patients’anxiety are knowledge level,the influence of disease on family life and career,and pain after operation.The main factors affecting patients’depression are course of disease,worry about prognosis,influence of disease on family career life and status of anal function.2.Before intervention,there were no significant differences in anxiety,depression,pain and quality of life scores between the two groups(P<0.05).After intervention,the scores of anxiety,depression and pain in the experimental group were lower than those in the control group,and the degree of reduction was more significant than that in the control group,with statistical significance(P<0.05);after a month of follow-up,the scores of quality of life in the experimental group were significantly higher than that in the control group,with statistical significance(P<0.05).Conclusion:1.Anxiety and depression exist in patients with complex anal fistula after operation.Prognostic anxiety,post-operative pain,knowledge understanding,the impact of disease on family life and career,course of disease and anal function are the influencing factors of anxiety and depression in patients with complex anal fistula after operation.Therefore,medical staff should timely assess the psychological status of patients,according to their psychological characteristics and influencing factors,through various ways,ways and positive effects to reduce or eliminate patients’negative emotions,so as to achieve the best psychological state of treatment and rehabilitation,and promote post-operative recovery.2.Focused solution model can significantly improve anxiety and depression in patients with complex anal fistula after operation,relieve pain and improve the quality of life of patients,which is worthy of clinical application. |
