| ObjectiveThis article includes two parts of clinical and basic research.The clinical analysis and effectiveness of clinical observation of platelet-rich plasma(PRP)in the treatment of nonunion provides a clinical basis for platelet-rich plasma for the treatment of nonunion.Through animal experiments,the effects of platelet-rich plasma containing traditional Chinese medicine on the treatment of non-continuous rat model were explored,and the potential mechanism of PRP in promoting bone formation and vascularization in bone healing was explored.MethodsReview of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from December 2016 to January 2019,using PRP to treat 18 patients with nonunion,collecting the gender,age,fracture causes,initial fracture treatment,nonunion time,General information such as the final treatment.Outcome indicators were collected for clinical healing and imaging data.The imaging criteria of external epiphyseal imaging,fracture line imaging score and bone healing time,VAS pain score,and complications were obtained according to imaging findings.The rat model of nonunion was established in animal experiments:the rats were divided into 4 groups,including Chinese medicine PRP,common PRP group,Chinese medicine group and blank control group.At 8 weeks after modeling,the rat model of nonunion was treated with PRP.The X-ray was observed and histomorphology was used to observe the outcomes of osteogenic differentiation and vascularization.ResultsA total of 18 patients with nonunion who underwent PRP treatment were retrospectively analyzed.17 patients had bony healing and 1 patient healed slowly.The average experience was 1.5±0.92 PRP,the average bone healing time was 237.65±59.22 days,the osteophyte imaging score was 5.89±1.08,and the fracture line imaging score was 4.67±1.14.All patients received clinical healing and clinical healing time.It is 103.39±39.04 days.ConclusionPlatelet-rich plasma is an effective,simple and safe method for the treatment of nonunion,which can promote bone healing of nonunion and is worthy of clinical promotion.Drug-containing platelet-rich plasma has a certain effect and provides a new direction for the development of platelet-rich plasma. |
