ObjectivesTo investigate the effectiveness of the collaborative work mode of medical care based on nursing quality management,evaluating the optimized alarm response working mode to shorten the response time of Cardiac Monitor alarm,reducing the false alarm rate of Cardiac Monitor alarm and adverse events of monitoring alarm,the effect of monitoring the red alarm response rate is to provide a scientific basis for further improving and improving the management level of Cardiac Monitor alarm response.MethodsCombined with the actual clinical practice,this study used a Class Experimental Design,with patients from CCU wards in a third-grade hospital in Hubei Province between June to December 2018.,By comparing the medical care work model based on the quality of care management before the implementation at time between June to September 2018 with the time between September to December 2018,Evaluation index including the patient’s monitoring of different alarm response time;red alarm within 5s of treatment ratio;two groups of patients the rate of false alarms of Cardiac Monitor;the incidence of adverse events of Cardiac Monitor and alarm in two groups of patients,and the clinical application effect of Cardiac Monitor and alarm response mode.The general data questionnaire and related instruments(GE Cardiac Monitor)were used as evaluation tools to collect all the alarm data and alarm processing records of the patients for 7 consecutive days,and the data were managed and analyzed by SPSS22.0 statistical analysis software.The main statistical analysis methods include: the mean ± standard deviation,rate or composition ratio to describe the quantitative data and qualitative data respectively;using chi-square test or t-test to optimize the Cardiac Monitor and alarm response mode based on nursing quality management.Results(1)The red,yellow and blue alarm treatment time of the control group were(13.0±10.79)s,(23.67±3.32)s,(23.67±3.32)s;those of the test group were respectively(8.11±5.27)s,(13.10±2.50)s,(26.05±4.16)s.The results of two independent samples with t-test showed that the difference of red alarm time between the two groups was statistically significant(P<0.05),the difference of yellow alarm time was statistically significant(P<0.05),and the difference of blue alarm treatment time was not statistically significant(P>0.05).(2)The control group red alarm ≤ 5s,6 ~ 10 s,11 ~ 15 s,16 ~ 20 s,> 20 s response treatment ratio were 37.98%,20.02%,18.59%,16.09%,7.32%;test group red alarm ≤ 5s,6-The proportions of 10 s,11-15 s,16-20 s,and >20s response were 42.36%,22.58%,17.28%,12.60%,and 5.18%,respectively.The chi-square test showed that the two groups were ≤5s,6-10 s,16-20 s.There was a statistically significant difference between the proportions of >20s response and treatment(P<0.05),and there was no significant difference between the 11~15s response rates(P>0.05).(3)The false alarm rate of Cardiac Monitor in the control group was 83.70%,and the false alarm rate of Cardiac Monitor in the test group was 38.55%.The results of chi-square test showed that the difference in false alarm rate between the two groups was statistically significant(P<0.05).(4)There were two adverse events occurred in the control group during Cardiac Monitor.The experimental group did not occur adverse events during Cardiac Monitor.There was no significant difference between the two groups(P>0.05).Conclusions(1)The optimized Cardiac Monitor and alarm response working mode is coordinated by the multi-disciplinary approach of medical technology,and the electrocardiographer is the main body of monitoring and alarm management,which shortened the response time of the alarm response,improved the proportion of the red alarm response,and reduced the false alarm rates of the Cardiac Monitor.The rates and the incidence of clinical adverse events have achieved good results,reducing the workload of nurses on the monitoring and alarm response,so that nurses have more time to engage in other nursing work,which is more conducive to the improvement of nursing quality.(2)The optimization of Cardiac Monitor and alarm response mode based on nursing quality management can be continuously improved in clinical practice,and targeted implementation according to the actual situation of each departments,so as to better promoting the patient’s Cardiac Monitor alarm response management level.
