Part Ⅰ Diffusion Tensor Imaging of Brain in Patients with unipolar depressionObjective To study the changes and significance of fractional anisotropy fraction(FA)in the prefrontal lobe and anterior cingulate gyrus of patients with unipolar depression.Methods 30 patients with unipolar depression(unipolar depression group)and 30 normal persons(healthy control group)were scanned with 1.5T GE magnetic resonance.DTI images of unipolar depression group and healthy control group were collected.Routine sequence and DTI sequence were scanned to exclude other intracranial lesions.The anisotropic scores of FA image were measured by hand.The difference of anisotropy scores between depression group and healthy group was compared.Result1.All subjects excluded intracranial organic lesions.All the collected images were clear and without distortion.2.The FA values of left superior frontal gyrus,middle frontal gyrus,inferior frontal gyrus and anterior cingulate gyrus were 0.402(+0.026),0.364(+0.020),0.281(+0.036),0.445(+0.029)for males and 0.411(+0.024),0.360(+0.011),0.293(+0.018)and 0.436(+0.016)for females,respectively.The FA values of inferior frontal gyrus and anterior cingulate gyrus were 0.397(+0.026),0.353(+0.016),0.283(+0.013),0.439(+0.029),while those of right superior frontal gyrus,middle frontal gyrus,inferior frontal gyrus and anterior cingulate gyrus were 0.398(+0.016),0.359(+0.012),0.293(+0.016)and 0.423(+0.013),respectively,with no significant difference(P>0.05).3.In unipolar depression group,the FA values of left superior frontal gyrus,middle frontal gyrus,inferior frontal gyrus and anterior cingulate gyrus were 0.391(+0.018),0.363(+0.323),0.270(+0.012),0.428(+0.023)in males and 0.390(+0.019),0.345(+0.025),0.280(+0.014),0.431(+0.011)in females,respectively.The FA values of the middle gyrus,inferior frontal gyrus and anterior cingulate gyrus were 0.370(+0.024),0.345(+0.039),0.270(+0.013),0.421(+0.020),while those of the right superior frontal gyrus,middle frontal gyrus,Inferior frontal gyrus and anterior cingulate gyrus were 0.375(+0.020),0.335(+0.026),0.280(+0.016)and 0.420(+0.013),respectively,with no siguificant difference(P>0.05).4.Compared witih the healthy control group,FA values of bilateral superior frontal gyrus,middle frontal gyrus,inferior frontal gyrus and bilateral anterior cingulate gyrus in the unipolar depression group were significantly lower(P<0.05).Conclusion The microstructures of prefrontal lobe and anterior cingulate gyrus are abnormal in patients with unipolar depression.These changes are involved in the occurrence of depression.DTI technology can be used to study depression,which has certain value in the diagnosis of unipolar depression.Part Ⅱ:Study of magnetic resonance spectroscopy in anterior cingulate gyrus of patients with unipolar depressionObjective To study the changes and significance of magnetic resonance spectroscopy in anterior cingulate gyrus of patients with unipolar depression.Methods 30 patients with unipolar depression(imipolar depression group)and 30 normal persons(healthy control group)were scanned by 1.5T GE magnetic resonance imaging.The sequence of brain magnetic resonance imaging was routine head sequence and MRS sequence.The scanning range was anterior cingulate gyrus.For subjects,such as N-acetyl aspartic acid(NAA),choline complex(Choline,Cho),and so on.The concentrations of creatine(Cr)and other metabolites in the anterior cingulate gyrus were measured and the ratios of NAA/Cr and Cho/Cr were calculated.The biometabolite ratios of depression group and healthy group were compared.Results1.There were no significant dififerences in NAA/Cr and Cho/Cr in the same cingulate gyrus between male and female in the healthy group,1.616(+0.057),1.318(+0.022),1.598(+0.043)and 1.320(+0.022)on the left side of male,1.615(+0.053),1.296(+0.036)on the right side of male,1.608(+0.034)and 1.301(+0.023)on the right side of female.(P>0.05)2.There were no significant differences in NAA/Cr and Cho/Cr in the same cingulate gyrus between male and female,1.581+0.035,1.300+0.029,1.582+0.034,1.305+0.036 on the left side of male,1.592+0.037,1.298+0.038 on the right side of male,1.593+0.035 and 1.296+0.029 on the right side of female.(P>0.05)3.The NAA/Cr of bilateral anterior cingulate gyrus in healthy control group and unipolar depression patients were 1.607(+0.051),1.581(+0.034)on the left side,1.611(+0.044),1.592(+0.036)and 1.319(+0.022)and 1.303(+0.032)on the right side,respectively.There were significant differences between unipolar depression group and unipolar depression group(P<0.05).Cho/Cr in the right anterior cingulate gyrus of the healthy control group and unipolar depression were 1.298(+0.030)and 1.297(+0.034),respectively,with no significant difference(P>0.05).Conclusion The abnormal metabolism of anterior cingulate gray matter is involved in the pathogenesis of unipolar depression.The decrease of NAA/Cr and Cho/Cr may be related to the neuropathology of unipolar depression.
