Objective:To analyze the guiding role of ultrasound on the choice of endotracheal intubation without cuffed tracheal tube model for children under 1 year old by measuring the diameter of the annular cartilage.Methods:A total of 240 children patients under 1 year old who accept Endoscopic abdominal surgery within an hour with tracheal intubation and general anesthesia in our hospital from November 2016 to November 2017,20 cases for each month age,were selected.According to whether children were treated with ultrasound to guide tracheal intubation,they were divided into ultrasound group and traditional group,120 cases in each group.Each group was divided into 4 subgroups according to the age of 10 cases per month:less than 3 months old group,3 to 5 months Age group,6 to 8 months old group,9 to 11 months old group.The tracheal intubation was finished with a cuffless tracheal tube.Type selection of endotracheal tube in ultrasound group depends on the results of ultrasonic measurement of the diameter of the annular cartilage,that is,the tracheal tube with the largest outer diameter closest to the measured value;the traditional group selected the tracheal tube according to the age formula.One intubation was successfully recorded as successful,otherwise it failed;the success rate of intubation,the number of attempts or replacements of tracheal intubation and the extubation complications,such as wheezing,pressure ventilation,re-intubation were recorded;the quality of postoperative resuscitation,such as extubation time,eye opening time in all children were assessed.Results:The success rate of intubation in the ultrasound group exceed the traditional group(P<0.05).The complications associated with endotracheal intubation in the ultrasound group were significantly lower than those in the traditional group(P<0.05);The complications associated with endotracheal intubation may occur in children of all month age groups,and the younger the age,the higher the risk of complications associated with tracheal intubation(P<0.05).There was no different after laparoscopic puncture needle placement in children patients from ultrasound group,HR 5 minutes after removal of the tracheal tube?MAP and PaCO2 levels between ultrasound group and traditional group(P>0.05);SpO2 levels in the ultrasound group and the traditional group decreased significantly at 5 minutes after extubation(P<0.05);But there was no difference between the ultrasound group and the traditional group(P>0.05).There was no significant difference in the time of extubation with each month age and waking time between the ultrasound group and the traditional group(P>0.05).The awakening time of<3 months in the ultrasound group was significantly shorter than that in the traditional group(P<0.05),but there was no significant difference in the other groups(P>0.05).The older the children were,the shorter the extubation time and awake time were.Conclusions:Ultrasound guides tracheal intubation in children under 1 year old can determine the type of cuffless tracheal tube,thereby improving the success rate of one intubation,and effectively reducing related complications.
