Objective:MRI examination of bilateral temporomandibular joints was performed in patients with temporomandibular joint structural disorder(ID)and healthy volunteers.The differences in disc length,post-disc and double plate thickness were measured by measurement.To deepen the clinician’s understanding of the morphological changes of the articular disc in the progression of ID disease,thereby guiding the clinical.Methods:1.From October 2016 to October 2018,20 patients with temporomandibular joint structural disorder in oral and maxillofacial surgery at the xx Hospital of Shanxi Medical University were selected as IDs due to unilateral or bilateral temporomandibular joint blast,joint pain,and mouth opening disorder.In the experimental group,the inclusion criteria were met(the diagnosis was based on the TMD biaxial diagnostic criteria).Ten asymptomatic healthy volunteers were selected as the control group and informed consent was obtained.All the patients who met the inclusion criteria were recorded in detail,according to the temporomandibular joint clinical.The specialist examination record form(see attached table)related project line bilateral TMJ specialist examination and record the results,collecting bilateral temporomandibular joint MRI images.2.According to the results of MRI examination,the ID experimental components were ADDR group and ADDWR group.The length of the bilateral articular disc,the posterior disc and the double plate of the control group,ADDR group and ADDWR group were measured and recorded in the oblique sagittal position.The thickness of the area was compared with that of the control group and the ADDR group and the ADDWR group.The data measured in this study,including the length of the articular disc,the thickness of the posterior disc,and the thickness of the double plate area of the articular disc,were measured by mean ± standard deviation.Multi-group mean comparison was performed by analysis of variance.LSD was used to perform multiple comparisons of multiple means.The test level was used to analyze the specific changes of the length of the articular disc,the length of the articular disc and the thickness of the double plate in patients with different disease progression ID.Provide a reference for the development of treatment plans.Results:1.Measurement of joint length: The mean value of the control group was11.34±1.23 mm,and the mean value of the ADDR group was 9.65±1.49 mm,the average value of the ADDWR group is 7.96±2.14 mm.The results of variance analysis between the three groups showed that there were differences among different groups.The LSD multiple comparison results showed that the length of the articular discs of the ADDR group and the ADDWR group were different compared with the control group,which showed a shortened change.There was a difference in the length of the disc length between the ADDWR group and the ADDR group,and the joint length measurement in the ADDWR group was lower than that in the ADDR group.2.In the thickness measurement of the posterior disc,the mean value of the control group was 2.75±0.11 mm,the mean value of the ADDR group was 3.24±0.39 mm,and the mean value of the ADDWR group was 2.65±0.41 mm.The results of variance analysis between the three groups showed that there were differences among different groups.The multiple comparison results of LSD showed that the thickness of the posterior disc was different between the control group and the ADDR group,and the measured thickness of the posterior band thickness in the ADDR group was higher than that of the control group.There was a difference in the thickness of the articular disc between the ADDR group and the ADDWR group.The measured value of the ADDR group was higher than that of the ADDWR group.There was no significant difference between the control group and the ADDWR group.There is a difference in the thickness of the posterior disc.3.In the thickness measurement of the double plate area of the articular disc: the mean value of the control group was 3.68±0.16 mm,the mean value of the ADDR group was 4.68±0.91 mm,and the mean value of the ADDWR group was 3.07±0.65 mm.The results of variance analysis between the three groups showed that there were differences among different groups.The multiple comparison results of LSD showed that the thickness of the double plate area of the control group and the ADDR group was different.The measured value of the double plate area of the ADDR group was higher than that of the ADDR group.The control group and the ADDWR group had different thickness of the articular disc double plate area,and the measured value of the ADDWR group was lower than that of the control group;the thickness of the double plate area of the ADDR group and the ADDWR group was different,and the measured value of the ADDR group was higher than that of the control group.Conclusion:1.In the progress of ID course,with the extension of the disc displacement distance and time,the change of the disc shape is more obvious,which can be mainly represented by the shortening of the length of the disc,and the greater the degree of disc displacement,the more obvious the disc shortening.2.The morphological changes of the articular disc may occur at the beginning of the ID lesion,which may be manifested as an increase in the thickness of the posterior disc and the double plate region,with the most significant change in the double plate region.3.MRI can accurately reflect the changes of the length and thickness of the articular disc in the progression of ID disease,and can accurately quantify the morphological changes of the articular disc,which has a wide range of clinical application value.
