Objective: To evaluate the efficacy and prognosis of laparoscopic treatment of placenta increta in normal position,in order to provide a basis for exploring a conservative surgical approach that is efficient,feasible,and retains fertility.Methods: The clinical data of patients who underwent placenta increta in our department were retrospectively analyzed,including: patient age,maternal parity,cesarean section,preoperative blood β-HCG value,timing of surgery,operation time,amount of surgical bleeding,The length of hospital stay,postoperative bloodβ-HCG decline,menstrual recovery,and the pregnancy status of patients with fertility after follow-up.Results: The average operation time of the 11 patients with placenta increta was(115.45±27.70)min,the average blood loss was(115.45±107.46)ml,and the average postoperative operation was(7.09±3.59)days.Among them,4 patients hadβ-HCG decreased to normal after 3-5 days,the remaining patient who did not fall to normal recovered menstruation at 43.50(19)days.In this study,8 patients with fertility requirements,the first patient was found to have intrauterine adhesions 2 months after surgery,and adjuvant estrogen and progesterone treatment after hysteroscopic adhesion separation,28 months and 62 months after surgery.Full-term delivery of healthy newborns,the first of which is natural conception,the second is due to bilateral infertility of the fallopian tube effusion by in vitro fertilization-embryo transfer;the second case of 43-month postpartum delivery of healthy newborns;The patients were born at full-term 31 months after surgery,and 3 patients with re-pregnancy did not have placental implantation.Conclusions: Laparoscopic placenta removal is a new and effective conservative surgical procedure for the treatment of placenta increta.It is a minimally invasive procedure to preserve the uterus and preserve fertility.It is a worthwhile treatment,and further evaluation of its long-term complications requires further long-term,large-sample,prospective clinical studies.
