| Objective To study the efficacy of voice training combined with drugs in the treatment of vocal nodules and the risk factors associated with poor prognosis.Methods Sixty patients with vocal nodules were selected and divided into two groups according to the different treatment methods.Each group was 30 cases.Group A patients underwent voice training combined with drug therapy,and group B patients underwent drug therapy alone.Comparing the differences between the two groups of patients before and after treatment,the comparison included the following indicators: VHI,VAS,MPT,DSI,jitter,shimmer,NHR,stroboscopic laryngoscopy changes,evaluation of the efficacy of voice training combined with drug treatment of vocal nodules.In the treatment of patients in group A and B,4 weeks,the efficacy of stroboscopic laryngoscopy was evaluated for 4 weeks,and the disappearance and cure of the nodules in the two groups were evaluated.At the end of treatment,all patients were divided into a cure group and a non-cure group according to the cure condition observed by stroboscopic laryngoscope.They were recorded as group C and group D respectively.The general situation of the two groups was compared to explore the risk factorsassociated with poor prognosis of vocal cord nodules.Results 1)There were no significant differences in the pre-treatment age,duration,VHI,VAS,MPT,DSI,jitter,shimmer,and NHR scores between the two groups(P>0.05).The VHI,VAS,jitter,shimmer and NHR values of the two groups were significantly lower than those before treatment.The MPT and DSI values were significantly higher than before treatment,and there was statistical difference(P<0.01).The values of VHI,VAS,jitter,shimmer and NHR in group A were significantly lower than those in group B.The MPT and DSI values were significantly higher than those in group B(P<0.01).2)Stroboscopic laryngoscope: After 4weeks of treatment,20 patients(66.7%)in group A were cured,6 patients(20%)improved,and 4 patients(13.3%)were ineffective.The overall improvement rate was 86.7%.Group B(46.7%)was cured,7 cases(23.3%)improved,9 cases(30%)were ineffective,and the total improvement rate was 70%.After 8 weeks of treatment,24 cases(80.0%)were cured in group A,4 cases were cured.(13.3%)improved,2 cases(6.7%)were invalid.Group B(66.7%)was cured,4(13.3%)improved,and 6(20%)were ineffective.The total effective rate of group A was93.3%,and the total effective rate of group B was 80.0%.The total effective rate of group A was higher than that of group B,and the difference was statistically significant(P<0.05).3)Multi-factor analysis showed that the course of disease and speaking habits(like shouting)arerisk factors that affect the conservative treatment of vocal nodules.Conclusion 1)voice training combined with drug therapy and simple drug therapy are effective for the treatment of vocal nodules.The combination of voice training and drug therapy for vocal nodules is more effective and can shorten the treatment cycle.It is worthy of clinical use.2)The course of the disease and the habit of speaking(like shouting)are independent risk factors that affect the conservative treatment of vocal nodules. |
