| ObjectiveThis study investigated the factors affecting the incidence of measles in the floating population in Shenzhen,and provided a basis for the development of a scientific and rational program for the prevention and control of measles disease.At the same time,the research on the antibody level of measles in healthy population of some floating population in Shenzhen will provide scientific warning information for the prevention and control of measles.MethodStudy on the risk factors of measles among floating population: Using case-control study design,case group were randomly selected according to the age of 7 months and below,8 months to 14 years,15 years and above;the case group selected 198 cases of measles cases in the floating population of Bao’an District reported by the Shenzhen legal infectious disease reporting system from 2014 to 2018;the 1:2 pairing method was used to match the age and gender,and the corresponding control group was selected.The investigator conducted a questionnaire survey on the subjects.Analysis using conditional logistic regression.Study on measles immunization status in healthy population: Using current research design,taking non-resident population living in Bao’an District for more than six months as the research object,in the health check-up population of hospitals and preventive health centers,through the convenient sampling,the qualified survey subjects are selected,and the survey is conducted.After the subject agreed,the subject was collected 3 ml of venous blood,and the investigator conducted a questionnaire survey;the collected blood samples were subjected to Enzyme-linked immunosorbent assay(ELISA)which performed to calculate the positive rate and geometric mean concentration(GMC)of measles IgG antibody.Categorical variables were compared using χ2 test and unconditional logistic regression analysis.Quantitative variables were analyzed by logarithmic conversion of antibody concentration by variance.The homogeneity test decided to use a rank sum test or t test.The definition of floating population is: Non-Shenzhen household registration,and living in the current residence is more than 6 months.Results1.Risk factors for measles in floating population: Among children aged 7 months and younger,medical history in 7 to 21 days(OR=4.89,95%CI:1.92~12.47,P=0.001)and the history of exposure to fever and rash(OR=8.98,95%CI:3.64~22.12,P<0.001)were risk factors,while the pregnancy cycle is 37~40 weeks(OR=0.09,95%CI:0.02~0.46,P=0.004)was a protective factor.Among the 8 months to 14 years old,the history of medical history within 7 to 21 days(OR=20.20,95%CI:3.53~116.00,P=0.001),got in touch with fever or rash patient(OR=76.00,95%CI:8.15~709.20,P<0.001)and living in humid room(OR=72.10,95%CI:1.94~268.00,P=0.020)were risk factors,and the history of measles vaccination(OR=0.06,95%CI:0.01~0.60,P=0.028)and mother’s high level of education(OR=0.20,95%CI:0.05~0.76,P=0.019)were protective factor.Among people aged 15 and over,medical history in 7 to 21 days(OR=6.37,95%CI:2.65~15.29,P<0.001),went to crowded places in 7 to 21 days(OR=4.45,95%CI:1.59~12.41,P=0.004)and room humidity(OR=28.14,95%CI:4.83~164.10,P<0.001)are risk factors,and room ventilation(OR=0.32,95%CI:0.11~0.91,P=0.032)was a protective factor.2.Measles immunization status of some healthy population in the floating population: A total of 1,560 people were tested,and the positive rate was 92.82%.The geometric mean concentration of the antibody was 694.41 mIU/ml.Multivariate logistic regression analysis of factors affecting antibody positive rate showed that the susceptibility of measles was relatively low in host children(OR=0.17,95%CI:0.05~0.54,P=0.002),and the susceptibility to measles was relatively high in people aged 16-24 years(OR=8.14,95%CI:5.02~10.30,P=0.031).Logistic regression analysis of factors affecting high-concentration measles antibody in children aged 7~15 years showed that the first needle of measles at 8~≤9 months was not conducive to the formation of high-concentration measles antibody(OR=4.80,95%CI:1.60~11.40,P=0.012);the age of 13-15 years was not conducive to high Maintenance of measles antibody concentration(OR=6.40,95%CI:3.50~11.80,P=0.016).Conclusion1.The risk factors for measles in infants aged 7 months and younger were 7 to 21 days of history and history of fever and rash exposure,while the protective factor was pregnancy period in 37~40 weeks;the risk factors for measles in the population of 8 months to 14 years old was the history of medical treatment in 7 to 21 days,got in touch with fever or rash patient and the humidity of the living room,while the protective factor was the mother’s high level of education;The risk factors for measles in population aged 15 and over was medical history in 7 to 21 days,went to densely populated places in 7 to 21 days and living in humid rooms,and the protective factor was room ventilation.2.Among some healthy population in the floating population hostage children had relatively low susceptibility to measles,but aged 16 to 24 years had relatively high susceptibility to measles;the first needle of measles at 8~≤9 months was not conducive to the formation of high-concentration measles antibody. |
