Serum Levels Of Leptin And Adiponectin In Patients With HBV-related Chronic Liver Disease And The Correlations With Liver Stiffness Measurement

Objective:To observe the serum levels of leptin(LP),adiponectin(ADN)and liver stiffness measurement(LSM)in patients with HBV-related chronic liver disease during the process of liver fibrosis,and to analyze the correlations between LP,ADN and LSM.Methods:Collecting the general clinical data and serum samples of 10 patients with chronic hepatitis B and 45 patients with cirrhosis of hepatitis B(According to Child-Pugh score,they were divided into 3 groups:Child A,Child B and Child C,with 15 people in each group)and 10 healthy persons as control group.The levels of LP and ADN in serum were measuredbyenzyme-linkedimmunosorbentassay(ELISA),liverstiffness measurement(LSM)was measured by FibroScan.Collecting the data of liver function,coagulation,abdominal color ultrasound.To observe the serum levels of LP,ADN and LSM in all samples,and analyze the correlation between them.Results:1.The serum levels of LP in Child-Pugh group[(9.48±1.86)μg/L,(12.21±1.46)μg/L,(16.46±0.64)μg/L]were significantly higher than that in chronic hepatitis B group and control group[(6.90±1.29)μg/L,(3.96±0.55)μg/L,P<0.01],increased in chronic hepatitis B group than that in control group(P<0.01).In cirrhosis patients,the serum levels of LP were increased sequentially with the increase of Child-Pugh score(P<0.01).2.The serum levels of ADN in chronic hepatitis B group were lower than that in control group[(21.26±4.24)μg/mL vs(70.77±7.85)μg/mL,P<0.01];the serum levels of ADN in Child A、Child B group[(25.37±9.31)μg/mL,(44.86±11.80)μg/mL]were lower than those in control group(P<0.01);the concentration of ADN in Child C group was lower than that in control group,but the difference was not significant[(65.55±7.35)μg/mL vs(70.77±7.85)μg/mL,P>0.05].The levels of ADN were increased gradually in liver cirrhosis with Child group,and the difference was statistically significant between the two groups.3.The LSM value of chronic hepatitis B group was higher than that of control group[(7.7±0.9)Kpa vs(3.2±1.0)Kpa],but the difference was not statistically significant(P>0.05).The LSM value of Child A group and Child B group and Child C group in liver cirrhosis[(20.3±9.1)Kpa、(29.3±11.7)Kpa、(51.6±13.9)Kpa]was significantly higher than that in chronic hepatitis B group and the control group[(7.7±0.9)Kpa,(3.2±1.0)Kpa,P<0.05],and that in liver cirrhosis groups was increased significantly in turn(P<0.05).4.In all the samples,there was a positive correlation between the serum levels of LP and LSM(r_s=0.828,P<0.01).There was a positive correlation between ADN and LSM in patients with liver cirrhosis(r_s=0.742,P<0.01).Conclusion:1.The serum levels of leptin in patients with HBV-related chronic liver disease increased gradually with the severity of fibrosis.2.The levels of serum ADN were decreased in patients with HBV-related chronic liver disease,but it showed an increasing trend in liver cirrhosis with the increase of liver cirrhosis grade.3.The LSM value in patients with HBV-related chronic liver disease increased gradually with the severity of fibrosis,and it had a positive correlation with serum levels of leptin,and it had a positive correlation with serum levels of ADN in patients with cirrhosis.
