Objective:1.Improving theoretical basis for avoiding perforation risk in clinical maxillary sinus elevation by analyzing the relationship between the shape of the maxillary sinus wall and the perforation rate of maxillary sinus.2.Analyzing the possible affecting factors in morphological typing of the maxillary sinus floor morphology,such as sex,the maxillary sinus is on the left or righ and ages.3.Analyzing the relative factors in maxillary sinus elevation operation include remaining bone height,maxillary sinus floor mucosa thickness and tooth missing time.Method:1.According to the inclusion criteria and exclusion criteria,a total of 47 patients underwent maxillary sinus internal elevation.Using the same device to obtain preoperative and postoperative CBCT images of patients in one experienced imaging physician,and images were processed by Adobe Photoshop CS5 software to obtain preoperative and postoperative coincident images of the maxillary sinus elevation.AutoCAD 2014 software was used to process the image to obtain the intersection point between the implant and the maxillary sinus floor wall before surgery,and the tangent point was used to obtain the tangent Angle of the maxillary sinus floor wall before surgery.Grouping the acquired angles into each 10° one group.Analyzing in statistics to the data by SPSS 22.0.Surgery were performed by the same experienced implant doctor.2.Analyzing the maxillary sinus floor morphology,factors that may affect the morphology of the maxillary sinus wall,such as the maxillary sinus is on the left or righ,gender,age.Discussing effect of the related factor for perforation rate in maxillary sinus,which included remaining bone height,maxillary sinus floor mucosa thickness and tooth missing time.Results:47 subjects were included,aged 24-67 years,average(54.7±13.61)years old,containing 28 male patients and 19 female patients,referring to 21 cases of left maxillary sinus and 26 cases of right maxillary sinus,with a total of 47 implants.Among 4 cases of perforation of maxillary sinus.(1)A significant difference was found in the perforation rate in maxillary sinus of different types of maxillary sinus wall morphology(P<0.05),the perforation rate is A type(-20° to-10°)>B type(-10° to 10°)>C type(10° to 30°).(2)The number of cases of B type and C type was significantly more than that of the number of A.(3)No significant difference in the morphology of the maxillary sinus wall between different sexes(P>0.05).(4)No significant difference in the classification of the maxillary sinus wall between the maxillary sinus on the left or right side(P>0.05).(5)No significant difference in the morphology of the maxillary sinus wall at different ages.ConclusionA significant effect in different forms of the maxillary sinus on the rate of perforation in the maxillary sinus wall.The maxillary sinus wall is mostly B and C,and A is less.The factors of gender,ages,whether the maxillary sinus is located on the left or right side,had no significant effect on the morphology of the maxillary sinus wall.
