Auxiliary Diagnostic Value Of Coagulation Function Indicators And Platelet Parameters In Pre-eclampsia

Objective:To investigate the auxiliary diagnostic value of coagulation function indicators and platelet parameters in pre-eclampsia.Methods:We select 325 pregnant women who gave birth from May 2016 to September2018,including 46 cases of gestational hypertension,48 cases of mild pre-eclampsia,131 cases of severe pre-eclampsia,and 100 normal pregnant women as controls.Changes of coagulation function indicators [prothrombin time(PT),activated partial thromboplastin time(APTT),international normalized ratio(INR),fibrinogen(FIB)]and platelet parameters [platelet counts(PLT),mean platelet volume(MPV),platelet distribution width(PDW),platelet crit(PCT),and platelet counts/platelet mean volume(PLT/MPV)] and clinical data of the four groups of subjects are recorded and analyzed in detail on the first day after hospitalization.Statistical processing is performed through SPSS 24.0 software.Results:1.There are no significant differences in age and parity between the four groups of pregnant women(P>0.05);There are significant differences in delivery gestational week,BMI,systolic blood pressure,diastolic blood pressure,coagulation function indicators,platelet parameters,and neonatal weight(P<0.05).2.By calculating the area under the ROC curve: APTT,MPV,PDW have auxiliary diagnostic value for pre-eclampsia(AUC=0.61、0.63、0.66;P<0.01);APTThas auxiliary diagnostic value for pre-eclampsia severity(AUC=0.67;P<0.01);combined detection of APTT、PT、INR、FIB、PLT、MPV、PDW、PCT and PLT/MPV have a higher auxiliary diagnostic value for pre-eclampsia and its severity(AUC=0.78、0.75;P<0.01).3.By logistic binary regression analysis,APTT,MPV,and PDW are risk factors for pre-eclampsia,APTT is a risk factor for severe pre-eclampsia.4.The retrospective test showed that combined detection of APTT、PT、INR、FIB、PLT、MPV、PDW、PCT and PLT/MPV have a higher correct rate in the auxiliary diagnosis of pre-eclampsia(73%).Conclusions:APTT,MPV,and PDW have value in the auxiliary diagnosis of pre-eclampsia and its severity,combined detection of all coagulation function indicators and platelet parameters in this study have clinical value in the auxiliary diagnosis of pre-eclampsia.
