Objective:To evaluate the subclinical function of the right ventricle in patients with obstructive sleep apnea syndrome(OSAS)early by two-dimensional speckle tracking(2D-STE),the analysis of early functional changes in the right ventricle of OSAS patients will provide a reliable basis for clinical intervention for early cardiac intervention in OSAS patients Methods:Thirty healthy subjects and 61 patients with OSAS by polysomnography were chosen as control group and study group.Among them,the study group were divided into three groups according to the apnea-hypopnea index(AHI),which were mild,moderate,and severe.Using general evaluation of right ventricular function parameters such as right ventricular end diastolic diameter(RVDD),right ventricular anterior wall(RVAW),right ventricular fractional area change(FAC),tricuspid annular plane systolic excursion(TAPSE),pulmonary artery systolic pressure(PASP),tricuspid annulus early diastolic peak velocity(E),atrial diastolic peak velocity(A)yielded E/A ratio,the deceleration time(DecT)of E wave were calculated and tissue Doppler measurement of tricuspid annulus systolic peak velocity(s’),tricuspid annulus early diastolic peak velocity(E’),atrial diastolic peak velocity(A’),etc.,and the right ventricular function was evaluated by two-dimensional speckle tracking technology,and combined with the blood biochemical indicators of the subjects,whether it was related to right heart function.Results:With the severity of OSAS disease,the tricuspid annulus systolic displacement(TAPSE)and E/A values gradually decrease,and the right ventricular global longitudinal strain(RVGLS)and right ventricular lateral wall longitudinal strain(RVLLS)are also reduced with severity,and they were statistically significant compared with the control group(P<0.01).But right ventricular septal longitudinal strain(RVSLS)no significant difference.The E/E’ ratio and the DecT value also increase proportionally with the severity of the disease.Triglyceride(TG)higher,TAPSE is reduced,while high-density lipoprotein(HDL)lower,TAPSE also lower.Moreover,and both compared with triglycerides greater impact on right ventricular function in patients with OSAS.Conclusion:RVGLS and RVLLS are gradually reduced with disease severity and are more sensitive than TAPSE.Therefore,two-dimensional speckle tracking technology can evaluate subclinical right ventricular dysfunction in OSAS patients.
