Objective:To investigate the potential relationships between DKI-derived parameters using whole-tumor volume histogram analysis and clinicopathologic prognostic factors in patients with rectal adenocarcinoma.Method:122 consecutive patientswhounderwent MRIexamination withrectal adenocarcinoma were retrospectively evaluated.Among them,48?39.3%?were female and74?60.7%?were male,with an average age of 60?42-81?years.The quantitative parameters including D,K and conventional ADC were measured using voxel-based histogram analysis for whole-tumor volume.Student's t-test,receiver operating characteristic curves,and Spearman's correlation were used for statistical analysis.Results:1.Significant negative correlations were observed between the 25th,50th percentiles and mean values of ADC and D,as well as ADC10th,with tumor T stages?P<0.05?.Similarly,there were statistically significant differences between MD values in different histopathological grades?P<0.05?,andnegatively correlated?r=-0.210,P<0.05?.2.Almost all the percentiles indexs of K were correlated positively with nodal involvement,higher histological grades,the presence of LVI and CRM?P<0.05?,with the exception between K10th,K90th0th and histological grades.And the k90th0th value of the advanced group was significantly higher than that of the early group?r=0.234,P<0.05?.3.Lower 75th and 90th percentiles of ADC and D values were also correlated inversely with nodal involvement?P<0.05?.In all histogram indicators,MK showed relatively higher area under the curve?AUC?and higher specificity than other percentiles for differentiation of lesions with nodal involvement.4.The index ki-67 were also negatively correlated with MD,ADCmean,D90th0th and ADC90th?r=-0.342-0.234,P>0.05?.Conclusion:DKI metrics with whole-tumor volume histogram analysis,especially the K parameters,were associated with important prognostic factors of rectal cancer.
