Purpose: Hypoxia-inducible factor 1-α(HIF1-α)is an important transcriptional regulator of hypoxia cells.It regulates the transcription of downstream genes,counteracts cell dysfunction caused by hypoxia,enhances glycolysis metabolism,and regulates cell biological process.This article detects the expression of HIF1-α in bladder urothelial carcinoma to study the correlation between HIF1-α expression level and clinical pathological parameters and patient prognosis,and provide a reference for the prognosis of bladder cancer patients with radical surgery.METHODS: Eighty-six patients with bladder urothelial carcinoma who underwent radical surgery were collected from the Affiliated Hospital of Qingdao University.Immunohistochemical staining was performed on 84 bladder cancer tissues to detect the expression of HIF1-α protein.We analyzed the expression of HIF1-α in bladder cancer patients and its age,sex,and tumor differentiation.Correlation between clinical and biological parameters such as the maximum diameter of the tumor,the number of tumors,and the presence or absence of vascular and neurological invasion.The prognostic effects of these factors were studied using the Kaplan-Meier method and the Cox proportional hazard regression model.RESULTS: There are 84 bladder cancer patients in total.72 patients were male,of which42 were negative for HIF1-α and 30 were positive for HIF1-α.12 patients were females,11 of which were negative for HIF1-α and 1 for HIF1-α,the difference was statistically significant(P < 0.05).The mean age was 68.9±10.8 years.70 cases were low grade carcinoma,of which 44 cases were negative for HIF1-α and 28 cases were positive for HIF1-α.There were 12 cases of high grade carcinoma,9 cases of HIF1-α negative expression and 3 cases of HIF1-α positive expression,the difference was not statistically significant(P>0.05).There were no vascular invasion in 73 cases,45 cases of HIF1-αnegative expression,28 cases of HIF1-α positive expression;there were 11 cases of vascular invasion,8 cases of which was HIF1-α negative expression,3 cases of which was HIF1-α positive expression,the difference is not Statistically significant(P>0.05).There were 76 cases without neurological invasion,47 cases of which were negative for HIF1-α and other 29 cases were positive for HIF1-α.There were 8 cases of neurological invasion,of which 6 cases were negative for HIF1-α and 2 cases were positive for HIF1-α.The difference was not statistically significant(P>0.05).The maximum diameter of the tumor with HIF1-α negative expression was 4.5±2.0cm,and the maximum diameter of the tumor with HIF1-α positive expression was 4.0±1.6cm.The difference was not statistically significant(P>0.05).The number of tumors in patients with negative HIF1-αexpression was 1.9±1.5,and the number of patients with positive expression of HIF1-αwas 1.8±1.5.The difference was not statistically significant(P>0.05).The age of HIF1-αnegative expression patients was 67.4±11.5 years old,and the age of HIF1-α positive patients was 71.6±8.9 years old.The difference was not statistically significant(P>0.05).By univariate and multivariate analysis,HIF1-α positive expression was identified as an important risk factor for overall bladder cancer survival.Kaplan-Meier survival analysis showed that patients with positive HIF1-α expression had worse overall survival than patients with negative HIF1-α staining.Conclusion: The results of the study indicate that the positive expression of hypoxia-inducible factor 1-α is gender-specific in adult bladder urothelial carcinoma patients,and the positive rate of male patients is higher than that of female patients.The positive expression of hypoxia-inducible factor 1-α is an independent predictor of low survival rate in patients with bladder urothelial carcinoma who underwent radical surgery for bladder cancer.HIF1α expression can be used as a prognostic marker for patients with bladder urothelial carcinoma who underwent radical surgery,helping doctors choose the best treatment for each patient.
